Decrease in Cerebral and Cerebellar Gray Matter in Essential Tremor: A Voxel-Based Morphometric Analysis under 3T MRI

BACKGROUND AND PURPOSE Though routine neuroimaging is usually normal in essential tremor (ET) there is clinical evidence of widespread involvement of central nervous system. This study aimed at determining morphological changes in brain of patients with ET using voxel-based morphometry (VBM) analysis and also compare the subtypes of ET. METHODS Clinical and imaging data of 20 patients (5 women, 15 men; mean age 38.2 +/- 16.5 years) of ET were collected prospectively. To determine areas of gray matter (GM) atrophy, T1W volumetric MRI data of patients were compared with 17 matched control subjects using VBM analysis with Statistical Parametric Mapping 5. RESULTS Compared to controls, patients showed scattered areas of cerebral and cerebellar atrophy. Voxel-wise analysis of patients with and without head tremor showed widespread areas of GM atrophy in cerebral hemispheres of patients with head tremor. No difference was observed between the familial and nonfamilial subgroups of ET. Multiple regression analysis revealed relationship between GM atrophy and tremor severity score. CONCLUSIONS Patients with ET showed widespread areas of atrophy in both cerebellum and cerebral GM, which supports the current concept of the progressive and diffuse nature of ET. Patients with additional head tremor may represent a distinct subgroup of ET.

This study aimed to provide synthesised cost-effectiveness evidence of rituximab in RA treatment and found that it is cost-effective compared to other DMARDs, but not as a third-line therapy after the failure of biologics.

Air Pollution and Cardiovascular Disease Burden: Changing Patterns and Implications for Public Health in India

Summary: The study explores the link between air pollution-attributed cardiovascular disease (APACVD) burden and its contributing factors in India. It reveals an inverse relationship between the APACVD burden and human development index (HDI), but a positive correlation with economic development indicators such as new motor vehicle registration and functional factories. Economically developed states seem to contribute a higher share of the APACVD burden.

The results showed that laparoscopic cholecystectomy is more cost-effective than conservative management, and early laparoscopic cholecystectomy is more cost-effective than delayed laparoscopic cholecystectomy.

A multicentre study on grey matter morphometric biomarkers for classifying early schizophrenia and parkinson's disease psychosis

Summary: Psychotic symptoms are common in schizophrenia and Parkinson's disease (PD) patients. Altered grey matter structure in various brain areas and networks may contribute to their pathogenesis. This study investigated the similarities in psychotic symptoms between schizophrenia and PD, and found significant grey matter reduction in both PD and early schizophrenia. Classification algorithms based on grey matter values within specific networks showed good accuracy in differentiating early psychosis cases and fair performance in distinguishing PD patients with and without psychotic symptoms. Results suggest some common underlying mechanisms and potential biomarkers for identifying these conditions.

Summary: This study measured the olfactory bulb volume (OBV) and olfactory sulcus depth (OSD) using MRI in patients with Parkinson's disease (PD) and compared them with patients with progressive supranuclear palsy (PSP), multiple system atrophy (MSA), vascular parkinsonism (VP), and controls. The results showed that PD patients had significantly lower OBV compared to the other groups. However, there was no significant difference in OSD. The findings suggest that OBV estimation by MRI can aid in the diagnosis of PD.

Cost-effectiveness of tumor necrosis factor-alpha inhibitors: a systematic review and meta-analysis of cost-utility studies

Summary: This study systematically reviewed the cost-utility evidence of TNF-a-i treatment for rheumatoid arthritis (RA) and found that TNF-a-i is not cost-effective compared to other disease-modifying anti-rheumatic drugs (DMARDs) and conventional synthetic DMARDs for RA treatment. However, the results cannot be generalized due to high heterogeneity and low confidence in GRADE quality assessment.

NIMHANS neuropsychological battery for elderly in Parkinson's Disease patients: Validation and diagnosis using MDS PD-MCI task force criteria in Indian population

Summary: This study aimed to validate the NIMHANS Neuropsychological Battery for Elderly (NNB-E) in diagnosing subtle cognitive deficits in Parkinson's Disease (PD) patients and evaluate cognitive impairment compared to healthy controls. The results showed that NNB-E had good internal consistency and construct validity, and it was able to detect mild cognitive impairment in PD patients with high accuracy. The study also revealed that PD patients had poor cognitive performance, particularly in executive and attention domains.

Spectrum and Pattern of Movement Disorders in Patients with Sporadic Creutzfeldt-Jakob Disease

Summary: This study reviewed the clinical, radiological features, and outcome of 25 CJD patients with movement disorders. The most common movement disorders observed were myoclonus, ataxia, and parkinsonism, while chorea was the least common. MRI showed basal ganglia abnormality and cortical ribboning in the majority of cases. EEG abnormalities were seen in 21 patients, with triphasic waves and periodic sharp waves being the most common. Abnormal 14-3-3 assay was found in 2 out of 4 patients. Atypical presentations, such as ataxic, CBS-like, and choreiform, were also noted.

Association of Scrub Typhus in Children with Acute Encephalitis Syndrome and Meningoencephalitis, Southern India

Summary: We systematically investigated 376 children with acute encephalitis syndrome (AES) in southern India and found that scrub typhus is an emerging common treatable cause of AES in this region. Diagnosing scrub typhus using IgM and PCR testing of blood and cerebrospinal fluid (CSF) showed that 16 cases were confirmed to be associated with AES, 55 cases were probable, and 16 cases were possible. Testing for IgM in CSF had a high sensitivity (93%) and specificity (82%) compared to PCR. These findings highlight the importance of routine testing for scrub typhus in diagnostic algorithms and suggest the potential promise of IgM screening of CSF for diagnosing AES caused by scrub typhus.

IOT based Sensor System for 24x7 monitoring movement disorder symptoms using machine learning

Summary: Parkinson's Disease (PD) is a challenge for automated monitoring due to its heterogeneous symptoms and progression. The current method involves physical records made by the patient or care giver about the response to medication, which adds burden to medical experts. Our aim is to use a combination of Non-Contact sleep monitoring system and wearable inertial measurement units to create a 24-hour monitoring system that can provide reliable data for better disease management with minimal intrusion.
