Summary: The study aimed to identify significant risk factors for major complications following contemporary CAS. Multivariable logistic regression found age, race, and hyponatremia significantly associated with stroke, while hyponatremia was associated with MI. Age, anemia, hypoalbuminemia, international normalized ratio, and emergency case were associated with death. These results suggest that preoperative risk stratification of CAS patients may not be straightforward, with each adverse event having its own risk factors.

However, when the etiology and location of aneurysms were more restricted, the incidence of RNF213 mutations in ICA-AN was higher than previously reported. This suggests that strict maternal vessel selection and pathological selection of aneurysm morphology may reveal an association between genetic mutations and ICA-AN development.

The choice between the two techniques should be based on the angioarchitecture of the aneurysms.

Endovascular treatment showed a higher rate of retreatment compared to microsurgery, and ruptured aneurysms had a higher retreatment rate, but similar rates of hemorrhage or rerupture compared to unruptured aneurysms.
