Summary: This study describes the clinical outcomes of an innovative minimally invasive endoscopic technique for the treatment of middle cranial fossa arachnoid cysts. The use of a mini endoscope allows for better examination and treatment of the cysts, resulting in a high success rate.

Summary: A 51-year-old woman with a known intrasellar arachnoid cyst for 23 years presented with progressive visual impairment. Magnetic resonance imaging showed significant enlargement of the lesion, extending into the left middle cranial fossa. The patient underwent cyst fenestration via the transsphenoidal approach. This is the first reported case of an intrasellar arachnoid cyst extending into the middle cranial fossa.

Summary: Arachnoid cysts in the middle cranial fossa are usually benign and asymptomatic, but in rare cases, they can rupture and lead to hemorrhages causing significant mass effect. We present two cases with subdural hemorrhage from middle cranial fossa arachnoid cyst, demonstrating very different clinical outcomes. Prompt diagnosis and appropriate surgical intervention are important in managing arachnoid cysts with hemorrhage, as these cases can have diverse clinical presentations and outcomes.

Summary: The majority of arachnoid cysts are congenital intracranial lesions that develop in the early embryonic stages. They are usually asymptomatic, but in rare cases, they can cause diplopia. We present a rare event of isolated fourth nerve palsy in a 56-year-old woman caused by an intracranial arachnoid cyst. This pathology is difficult to diagnose and treat.
