The aim of this study was to retrospectively investigate the efficacy of gamma knife radiosurgery for brain metastases from advanced gastric cancer (AGC) comparing whole brain radiotherapy (WBRT). Between January 1991 and May 2008, 56 patients with brain metastases from AGC, treated with GKR or WBRT, were reviewed to assess prognostic factors affecting survival. Most brain metastases were diagnosed based on MRI, both metachronous and synchronous brain metastases, adenocarcinoma and signet ring carcinoma were included, but excluded cases of gastric lymphoma. Fifteen patients with a median age of 54.0 years (range, 42-67 years) were treated with GKR: 11 were treated with GKR only, 2 with surgery plus GKR, 1 with repeated GKR, 1 with GKR plus WBRT, and the other 1 with WBRT plus GKR. Forty-one were treated with WBRT only. The median number of metastatic brain lesions was 3 (range, 1-15), and treatment involved 17.0 Gy (range 14-23.6 Gy), or 30 Gy with fractionated radiotherapy. The median survival after brain metastases for GKR treatment was 40.0 weeks [95% confidence interval (CI) 44.9-132.1 weeks] and WBRT was 9.0 weeks 95% CI, 8.8-21.9 weeks). The progression free survival of 15 GKR treated patients was 56.5 weeks (95% CI 33.4-79.5 weeks). The recursive partitioning analysis (RPA) (class 2 vs. class 3) and use of GKR were correlated with prolonged survival in univariate and multivariate analyses. Age, sex, pathology, leptomeningeal seeding, tumor size (a parts per thousand yen3 cm), extracranial metastases, single metastasis, chemotherapy, and synchronous metastases were not correlated with a good prognosis in both univariate and multivariate analysis. Based on our study, the use of GKR and RPA class 2 resulted in more favorable clinical outcomes in patients with brain metastases from AGC.

What we need to know about uncertainty in illness among people with primary malignant brain tumours: A mixed-methods systematic review

Soomin Hong, JuHee Lee, Jiyeon Lee, Jong Hee Chang, Chang Gi Park, Tae Hee Kim

Summary: This study aims to identify the characteristics of uncertainty in illness among people with primary malignant brain tumors through a mixed-methods systematic review. The results reveal that the progression of uncertainty in people with primary malignant brain tumors is complex and ambiguous due to physical, psychological, and social risk factors. The proposed model provides directions for nursing practice and future research.

Comparison of Single-Session, Neoadjuvant, and Adjuvant Embolization Gamma Knife Radiosurgery for Arteriovenous Malformation

Summary: This study retrospectively analyzed data from 453 AVM patients and found that combined neoadjuvant or adjuvant embolization with GKRS treatment had better outcomes compared to GKRS alone, reducing the risk of bleeding during the latency period. Additionally, the study also found that neoadjuvant embolization may be a negative predictive factor for complete obliteration of AVMs more than 36 months after GKRS.

Summary: This study designed a brain-machine interface (BMI) system that uses sensory-related neural signals for control and combines it with electrical stimulation as reward. Electrodes were used to record electrocorticographic (ECoG) signals from the whisker-related somatosensory cortex of rats, which were then converted into BMI signals to control the movement of a dot on the screen. The rats were trained for 7 days using operant conditioning with medial forebrain bundle (MFB) electrical stimulation to learn the task of moving the dot towards the desired position.

Summary: Focused ultrasound (FUS) is a promising non-invasive medical technology that stimulates the brain. This study investigated the effects of FUS-mediated blood-brain barrier (BBB) opening on hippocampal long-term potentiation (LTP) and cognitive function in a mouse model of Alzheimer's disease. The results showed that FUS-mediated BBB opening increased LTP and improved cognitive function.

Summary: This study investigated prognostic factors in adult glioma patients with leptomeningeal metastases. It found that chemotherapy and antiangiogenic therapy were associated with longer overall survival, while male sex and initial leptomeningeal metastases were associated with shorter survival.

Summary: This study investigated the prognostic impact of serum biomarkers in patients with newly diagnosed atypical meningiomas. The findings revealed that tumor size, surgical resection, and postoperative serum markers were associated with the progression/recurrence rate. Further studies are needed to validate their clinical significance and biological background.

Summary: Bilateral thermal capsulotomy with magnetic resonance-guided focused ultrasound (MRgFUS-capsulotomy) is a promising treatment option for treatment-refractory obsessive-compulsive disorder (OCD). This study investigated the effects of MRgFUS-capsulotomy on neural oscillations in OCD patients. The results showed that the high beta band power in certain brain areas decreased at 1 month and remained stable for 6 months after the procedure. The cortical connectivity of the high beta band also gradually decreased over time. Improvement in anxiety and depression symptoms was significantly correlated with changes in high beta band power.

A Single Injection of rAAV-shmTOR in Peripheral Nerve Persistently Attenuates Nerve Injury-Induced Mechanical Allodynia

Minkyung Park, Ha-Na Woo, Chin Su Koh, Heesue Chang, Ji Hyun Kim, Keerang Park, Jin Woo Chang, Heuiran Lee, Hyun Ho Jung

Summary: This study investigated the effect of injecting rAAV-shmTOR on the downregulation of mTOR and glial activation. The results showed that peripheral sensitization was gradually attenuated in the shmTOR group, and both p-mTORC1 and p-mTORC2 were downregulated in the DRG. The study suggests the potential application of interfering with the mTOR signaling pathway in pain treatment.

Summary: This study aimed to evaluate the effect of photodynamic therapy (PDT) and sonodynamic therapy (SDT) on glioblastoma (GBM), as well as the combination therapy of these two (SPDT). The results showed that PDT had an inhibitory effect on tumor volume, while the SDT group showed a slight increase. Light with sensitizers can inhibit GBM growth, but ultrasound had a weaker effect. Further studies are needed to investigate the safety parameters of ultrasound in GBM treatment.

Summary: The blood-brain barrier (BBB) acts as a barrier for molecules in passing from the blood to the brain. Focused ultrasound has emerged as a novel method for temporarily opening the BBB, allowing for delivery or removal of neurodegenerative proteins. This study aimed to review our BBB opening procedure using magnetic resonance guided images and identify specific patterns.

Long-term results on the suppression of secondary brain injury by early administered low-dose baclofen in a traumatic brain injury mouse model

Ji Young Park, Junwon Park, Jiwon Baek, Jin Woo Chang, Young Goo Kim, Won Seok Chang

Summary: This study aimed to investigate the potential therapeutic effects of early administration of baclofen in traumatic brain injury (TBI) mice. The results showed that baclofen treatment significantly reduced lesion volume and inhibited neuroinflammation, glial cell activation, and apoptosis. Furthermore, baclofen improved TBI motor and cognitive abnormalities. These findings suggest that early administration of low dose baclofen could be beneficial for TBI treatment.
