Screening for anorexia nervosa via measurement of serum leptin levels

Due to their sub-normally low fat mass, leptin levels in patients with acute anorexia nervosa (AN) are well below reference levels for age and sex-matched controls. This hypoleptinemia entails endocrinological and behavioral characteristics observed in AN patients during starvation. We aimed to study the appropriateness of hypoleptinemia as a diagnostic marker for AN by assessing sensitivity, specificity and likelihood ratios for different referral serum leptin levels for predicting anorexia nervosa and healthy leanness. For prediction, we additionally generated a score based on a multivariate logistic model including body mass index (BMI; kg/mA(2)) and leptin level. For this purpose, we measured leptin levels in 74 female patients with acute AN upon admission for inpatient or outpatient treatment. Adolescent and adult patients were recruited according to DSM-IV criteria from two multi-center studies. Additionally, leptin levels were measured in 65 female healthy, lean students. Mean serum leptin level was significantly decreased in patients with AN compared to underweight controls (0.87 +/- A 0.90 vs. 6.43 +/- A 3.55 mu g/L, p < 0.001). Leptin predicted AN independently of BMI; we confirmed a cutoff value in the range of 2 mu g/L as having both high specificity and sensitivity. Hypoleptinemia represents a state marker of acute AN and is useful for a laboratory-based diagnostic screening.

Summary: Despite limited knowledge about the benefit-risk ratio in clinical practice, a study monitoring children and adolescents treated with antidepressants and/or antipsychotics found that off-label use did not significantly increase the risk of serious adverse drug reactions. This suggests that closely monitored clinical settings can provide valuable insights bridging the gap between randomized controlled trials and real-world clinical settings.

Value of a web-based pediatric drug information system to prevent serious adverse drug reactions in child and adolescent psychiatry

Effects of lifestyle and educational bridging programs before bariatric surgery on postoperative weight loss-a systematic review and meta-analysis

Mirror Therapy in Patients with Somatoform Pain Disorders-A Pilot Study

Summary: Patients with chronic pain, especially those with somatoform pain disorders, may benefit from mirror therapy in terms of reducing pain intensity and modulating physiological parameters. This study showed that mirror therapy led to a reduction in pain intensity and increased sensitivity to cold stimuli among patients with somatoform pain disorders. It also revealed a decrease in the low-frequency band of heart rate variability, indicating a potential effect on autonomic nervous system regulation. However, further studies with larger sample sizes and control groups are needed to validate these findings.

Summary: This study investigated the relationship between drug dose and serum concentration of fluoxetine in children and adolescents and established the therapeutic serum reference range. The study found a positive correlation between dose and serum concentration of fluoxetine and its metabolite, with higher body weight and female sex associated with lower serum concentrations and therapeutic response. Most patients showed marked or minimal improvements during treatment, with no reported adverse effects.
