Summary: New therapies are urgently needed for ovarian cancer, and this review discusses the remarkable advances in identifying metabolic vulnerabilities and strategies for targeted therapy. The development of tumor-selective approaches, such as folate receptors and the proton-coupled folate transporter, as well as the discovery of drugs targeting key metabolic processes, show extraordinary potential for improving the lives of women with this devastating disease.

Summary: The delivery of therapeutic nucleic acids is a promising approach for treating both inherited and acquired diseases. Folic acid and its lipoconjugates, which have low immunogenicity, high tumor penetration, and high affinity for a wide range of tumor cells, are used for targeted delivery. Liposomal gene delivery systems using folate lipoconjugates are discussed, along with important development steps such as rational design and size control.

Summary: Recently, folate-coated pH-sensitive liposomes have been studied as a tool to enhance cellular uptake and antitumor activity of paclitaxel and doxorubicin. This study demonstrated the importance of folate functionalization in doxorubicin-loaded liposomes and showed their significant reduction in cardiac toxicity. This provides a promising drug delivery strategy for the treatment of breast tumors.

Summary: In this retrospective analysis, the safety and activity of metronomic chemotherapy (MC) based on topotecan or a combination of topotecan and cyclophosphamide were evaluated in advanced ovarian cancer patients. MC demonstrated clinically meaningful activity and good safety profile in this difficult population. The study also suggested that overweight or obese patients had significantly better outcomes with MC than patients with BMI <25 kg/m(2).

Summary: This study examined the role of ALDH1A1 expression in paclitaxel and topotecan resistant ovarian cancer cell lines. The results showed that knockout of ALDH1A1 gene had no effect on MDR1/P-gp expression and drug resistance in the PAC-resistant cell line, but resulted in a moderate decrease of BCRP and COL3A1 expression and weakened TOP resistance in the TOP-resistant cell line. Clonal selection of ALDH1A1 cells led to strong downregulation of BCPR and COL3A1 expression and overexpression of MDR1/P-gp, resulting in decreased resistance to TOP but increased resistance to PAC. Spheroids were more resistant than cells growing as monolayers, possibly due to different proliferation paces, spheroid density, ECM expression, and drug capacity to diffuse into the spheroid.

Summary: EP0057, an investigational nanoparticle-drug conjugate, in combination with paclitaxel has shown efficacy in treating recurrent EOC, with a recommended phase 2 dose of 15 mg/m(2) EP0057 every 2 weeks plus 80 mg/m(2) paclitaxel weekly. Although the observed overall response rate was not statistically better than historical control rate, EP0057 remains an interesting option for treatment of recurrent EOC.

Summary: Studies have shown that CHFR levels vary among ovarian cancers but are unlikely to be an independent biomarker for poor response to taxanes. Additionally, CHFR expression is not significantly associated with overall survival or time to progression in high grade serous ovarian cancer patients treated with platinum/taxane chemotherapy. Furthermore, there is no observed association between CHFR expression and paclitaxel sensitivity in patient-derived ovarian cancer xenografts.

Summary: The definition of platinum-resistant ovarian cancer has changed and standard treatment is limited in effectiveness. Combination treatment with antiangiogenics has shown better outcomes, while immune checkpoint inhibitors have modest results. Reliable biomarkers to predict treatment response are lacking. The optimal timing for platinum retreatment in platinum-resistant ovarian cancer remains a topic of debate.

Summary: High DIAPH1 levels are correlated with increased survival of ovarian cancer patients. Down-regulation of DIAPH1 expression decreases PTX sensitivity and reduces the concentration of stable microtubules. In addition, DIAPH1 enhances the effect of PTX on microtubule stability in a cell-free system. Thus, DIAPH1 increases the response of Ovca cells to PTX.

Summary: Migration of epiphyseal screws into growing physis is a known complication in tension band plating. This study investigated the technical details contributing to this complication in 10 patients. The length of the screw, proximity to the physis, trajectory angle, and correction rate were analyzed. Migrated plates had a closer starting point to the physis and a more divergent screw trajectory compared to uncomplicated plates. Osteopenic bone and pathological physis were identified as predisposing factors.
