Reactive oxygen species (ROS) are highly reactive oxygen-derived chemical compounds that are by-products of aerobic cellular metabolism as well as crucial second messengers in numerous signaling pathways. In excitationcontraction-coupling (ECC), which links electrical signaling and coordinated cardiac contraction, ROS have a severe impact on several key ion handling proteins such as ion channels and transporters, but also on regulating proteins such as protein kinases (e.g. CaMKII, PICA or PKC), thereby pivotally influencing the delicate balance of this finely tuned system. While essential as second messengers, ROS may be deleterious when excessively produced due to a disturbed balance in Na+ and Ca2+ handling, resulting in Na+ and Ca2+ overload, SR Ca2+ loss and contractile dysfunction. This may, in the end, result in systolic and diastolic dysfunction and arrhythmias. This review aims to provide an overview of the single targets of ROS in ECC and to outline the role of ROS in major cardiac pathologies, such as heart failure and arrhythmogenesis. Summary: Novel ICD programming has been shown to reduce mortality and morbidity due to appropriate or inappropriate ICD therapies in both secondary and primary ICD indications. The new programming strategy demonstrated significant reductions in mortality rate as well as rates of appropriate and inappropriate therapies, indicating its potential benefit in improving outcomes for patients with ICD and CRT-D.

Summary: This study compared clinical characteristics and in-hospital outcomes between Japanese and European takotsubo syndrome (TTS) patients, revealing differences in triggers and outcomes between the two ethnic groups. Despite worse in-hospital outcomes for Japanese patients, ethnicity itself does not appear to impact TTS patient outcomes. Machine learning analysis identified physical stressors as the most important prognostic factor for both Japanese and European TTS patients.

Infarct-Related Artery as a Donor of Collaterals in ST-Segment-Elevation Myocardial Infarction With Concomitant Chronic Total Occlusion: Challenge of the Double-Jeopardy Thesis

Summary: This study investigated the relationship between chronic total occlusion (CTO) in a non-infarct-related artery (NIRA) and adverse outcomes in ST-segment-elevation myocardial infarction (STEMI). The presence of CTO posed a threat to coronary blood supply, and the origin of intercoronary collaterals was associated with patient survival rate. The results showed that CTO collateralization from the IRA distal to the acute occlusion was associated with a better prognosis.

Summary: The study aimed to evaluate the association between common diseases and nocturnal hypoxemic burden in patients undergoing coronary artery bypass grafting surgery. The results showed that sleep-disordered breathing, obesity, and mild-to-moderate chronic obstructive pulmonary disease were significant predictors of increased nocturnal hypoxemic burden.

Insights into the Interaction of Heart Failure with Preserved Ejection Fraction and Sleep-Disordered Breathing

Summary: Heart failure with preserved ejection fraction (HFpEF) is a widespread disease with high morbidity and mortality, and sleep-disordered breathing (SDB) has been found to be closely associated with the development and progression of the obese HFpEF phenotype. Individualized therapeutic strategies and novel technologies like single-cell transcriptomics or CRISPR-Cas9 gene editing are needed to improve treatment outcomes.

Mechanistic target of rapamycin in regulating macrophage function in inflammatory cardiovascular diseases

Summary: mTOR, a mechanistic target of rapamycin, is an evolutionarily conserved pathway that plays a fundamental role in nutrient sensing, growth, metabolism, lifespan, and aging. Recent studies have highlighted the regulatory role of mTOR in innate immune responses and its involvement in the pathogenesis of cardiovascular diseases, especially in acute inflammation and atherosclerotic cardiovascular disease. This review also discusses mTOR's role in trained immunity, immune senescence, and clonal hematopoiesis, as well as its architecture and regulatory complexes.
