Pregnancy Complicated by Obesity: Midwifery Management

Obesity-related comorbidities such as gestational diabetes and hypertension have the potential to affect at least 25% of women in the United States. Midwives have been caring for and collaboratively managing these conditions in nonobese women for decades. Prenatal weight gain advice should be based on pregravid body mass index and aim for the lower end of the 1990 Institute of Medicine prenatal weight gain ranges. Obese women may require extra ultrasound and blood glucose testing during pregnancy. Pregnancy complicated by obesity may limit the place and style of birth. Midwives can integrate management techniques into the perinatal care of women whose body mass indices exceed 29 to reduce risk and future disease for mothers and newborns. Summary: Primiparous women with obesity can safely and effectively receive midwifery care at freestanding birth centers with a low risk for cesarean birth. There were no significant differences in perinatal outcomes between women with obese BMIs and normal BMIs, with the majority of women with obese BMIs experiencing uncomplicated perinatal courses and vaginal births.

Pregnant women's perspectives on integrating preventive oral health in prenatal care

Summary: Pregnant women in British Columbia generally support the integration of preventive oral care into routine prenatal services, with the establishment of a referral system seen as an acceptable strategy for providing integrated prenatal oral health care. However, there are differing views on how this integration can be achieved, and limited funding is acknowledged as a barrier to oral health care access.

An interprofessional model of care for oral health during pregnancy

Management of Labor and Delivery to Reduce Risk for Cesarean Birth in Women with Obesity

Summary: This review provides management guidelines for labor and birth for individuals with obesity and emphasizes methods for reducing cesarean birth risk. Research shows that a significant percentage of people in labor have obesity-related complications, which can be attributed to excessive adipose tissue producing adipokines that disrupt normal labor physiology and increase inflammation. However, individuals with obesity but without other comorbidities have high rates of uncomplicated pregnancies and lower cesarean section rates, especially if they are younger and have had previous children.

A Qualitative Study of Health Care Providers' Views on Integrating Oral Health into Prenatal Care

The results highlighted the importance of interprofessional collaboration as a critical factor for integrated care.
