Oropharyngeal Colostrum Administration in Extremely Premature Infants: An RCT

OBJECTIVE: To determine the immunologic effects of oropharyngeal colostrum administration in abstract extremely premature infants. METHODS: We conducted a double-blind, randomized, placebo-controlled trial involving 48 preterm infants born before 28 weeks' gestation. Subjects received 0.2 mL of their mother's colostrum or sterile water via oropharyngeal route every 3 hours for 3 days beginning at 48 to 96 hours of life. To measure concentrations of secretory immunoglobulin A, lactoferrin, and several immune substances, urine and saliva were obtained during the first 24 hours of life and at 8 and 15 days. Clinical data during hospitalization were collected. RESULTS: Urinary levels of secretory immunoglobulin A at 1 week (71.4 vs 26.5 ng/g creatinine, P = .04) and 2 weeks (233.8 vs 48.3 ng/g creatinine, P = .006), and lactoferrin at 1 week (3.5 vs 0.9 mu g/g creatinine, P = .01) were significantly higher in colostrum group. Urine interleukin-1 beta level was significantly lower in colostrum group at 2 weeks (55.3 vs 91.8 mu g/g creatinine, P = .01). Salivary transforming growth factor-beta 1 (39.2 vs 69.7 mu g/mL, P = .03) and interleukin-8 (1.2 vs 4.9 ng/mL, P = .04) were significantly lower at 2 weeks in colostrum group. A significant reduction in the incidence of clinical sepsis was noted in colostrum group (50% vs 92%, P = .003). CONCLUSIONS: This study suggests that oropharyngeal administration of colostrum may decrease clinical sepsis, inhibit secretion of pro-inflammatory cytokines, and increase levels of circulating immune-protective factors in extremely premature infants. Larger studies to confirm these findings are warranted.

Factors associated with the follow-up of high risk infants discharged from a neonatal intensive care unit

Summary: This retrospective cohort study aimed to identify factors associated with compliance to follow-up appointments among infants discharged from the neonatal intensive care unit (NICU). The study found that factors such as assisted reproductive technology usage, gestational age, birth weight, height, head circumference, and the number of siblings were associated with non-compliance. Additionally, Apgar scores, residential location, and certain complications were also found to be related to compliance. The study highlights the importance of these factors in determining follow-up compliance rates for infants.

Deep learning-based quantitative analyses of spontaneous movements and their association with early neurological development in preterm infants

Summary: This study aimed to develop a quantitative assessment method for spontaneous movements in preterm infants using a deep learning algorithm. The complexity and similarity indices of joint angles and angular velocities were compared between infants with different Hammersmith Infant Neurological Examination (HINE) scores. The results showed that complexity indices of joint movements were positively correlated with HINE scores, suggesting their potential as indicators of developmental outcomes in preterm infants.

Impact of Cerebellar Injury on Neurodevelopmental Outcomes in Preterm Infants With Cerebral Palsy

Summary: In this study, brain imaging findings and neurodevelopmental outcomes of preterm infants diagnosed with cerebral palsy were analyzed. The study found that high-grade cerebellar hemorrhage and/or atrophy accounted for 25% of cerebellar injuries in patients with supratentorial lesions. Patients with cerebellar injury showed lower scores in various domains of neurodevelopmental assessment and had a higher proportion of patients below the cutoff value. Poor neurodevelopmental outcomes, other than motor function, were associated with cerebellar injury, indicating the importance of evaluating the cerebellum in predicting functional outcomes of cerebral palsy patients.

Risk factors of early pulmonary hypertension and its clinical outcomes in preterm infants: a systematic review and meta-analysis

Summary: The aim of this meta-analysis was to determine the incidence and risk factors of early pulmonary hypertension (PHT) in preterm infants and evaluate its association with morbidities such as bronchopulmonary dysplasia (BPD), late PHT, and in-hospital mortality. The study found that early PHT is significantly associated with BPD, in-hospital mortality, and late PHT in preterm infants. Oligohydramnios and small-for-gestational-age are potential risk factors for early PHT in preterm infants.

Neurodevelopmental outcomes and volumetric analysis of brain in preterm infants with isolated cerebellar hemorrhage

A thickened formula reduces feeding-associated oxygen desaturation and bradycardia in preterm infants

Summary: This study aimed to investigate the effects of a commercial thickened formula on oxygen saturation and heart rate stabilization during oral feeding in preterm infants. The results showed that TF feeding significantly reduces the frequency of desaturation and bradycardia during oral feeding, without causing diarrhea or electrolyte abnormalities.

Association of newer definitions of bronchopulmonary dysplasia with pulmonary hypertension and long-term outcomes

Summary: This study evaluated the association between different definitions of bronchopulmonary dysplasia (BPD) and the occurrence of pulmonary hypertension (PHN) and long-term outcomes. The severity of BPD based on the 2019 NICHD definition was found to be associated with long-term outcomes and PHN at a postmenstrual age of 36 weeks in preterm infants.
