Molecular imaging methods such as positron emission tomography (PET) are increasingly involved in the development of new drugs. Using radioactive tracers as imaging probes, PET allows the determination of the pharmacokinetic and pharmacodynamic properties of a drug candidate, via recording target engagement, the pattern of distribution, and metabolism. Because of the noninvasive nature and quantitative end point obtainable by molecular imaging, it seems inherently suited for the examination of a pharmaceuticals behavior in the brain. Molecular imaging, most especially PET, can therefore be a valuable tool in CNS drug research. In this Perspective, we present the basic principles of PET, the importance of appropriate tracer selection, the impact of improved radiopharmaceutical chemistry in radiotracer development, and the different roles that PET can fulfill in CNS drug research.

Summary: This study compared the lesion detection rate of [Ga-68]Ga-PSMA-11 PET/CT, [Ga-68]Ga-NODAGA(ZOL) PET/CT, and [Tc-99m]Tc-MDP bone scan in assessing bone metastases in patients with advanced prostate cancer. The results showed that in the staging of skeletal metastases, [Ga-68]Ga-PSMA-11 PET/CT may detect more lesions than [Ga-68]Ga-NODAGA(ZOL) PET/CT and [Tc-99m]Tc-MDP bone scan.

Evaluation of Safety and Dosimetry of 177Lu-DOTA-ZOL for Therapy of Bone Metastases

Summary: The study evaluated the safety and dosimetry of a new therapeutic radiopharmaceutical, 177Lu-DOTA-ZOL, for bone metastasis through a series of SPECT/CT images and blood samples. Results showed fast uptake and high retention in bone lesions, with a stable average retention rate in tumor lesions after injection. The red marrow was identified as the dose-limiting organ for all patients, suggesting personalized dosimetry should be considered to prevent severe hematotoxicity.

Summary: Radiochemical conversion is crucial in radiopharmaceutical chemistry, measuring the efficiency of reaction by studying the transformation of components in a crude reaction mixture. On the other hand, radiochemical yield is more suited to determining the efficiency of the entire reaction process, including separation, isolation, filtration, and formulation.

Summary: In this study, a new hybrid chelator DATA(5m) was introduced for PET/CT imaging of SST2R-positive tumors. The [Ga-68]Ga-DATA(5m)-LM4 tracer showed excellent binding affinity in cell and mouse models, and accurately visualized tumor lesions in a patient.
