DIMETHYLARGININE - BIOMARKERS IN PROGRESSION OF KIDNEY DISEASE

Decreased nitric oxide (NO) production and/or impaired NO bioavailability may occur in patients with the chronic kidney disease (CKD), and could contribute to elevation of blood pressure, cardiovascular disease (CVD) and progression of renal injury in these patients. Free guanidino-methylated arginine residues occur endogenously as a result of proteolysis of post-translational methylated tissue proteins. The asymmetric dimethyl arginine (ADMA) is a competitive inhibitor of the nitric oxide synthase (NOS) enzymes. The kidney has a predominant role in ADMA elimination by combining two mechanisms; urinary excretion and metabolization of ADMA The degradation of ADMA is accomplished intracellularly by the enzyme dimethylarginine dimethylaminohydrolase (DDAH). ADMA is not only a uremic toxin, but also a strong marker of the endothelial dysfunction and atherosclerosis and a stronger independent predictor of all-cause mortality and cardiovascular outcome in patients with the chronic renal failure. There are at least four mechanisms that may explain the accumulation of ADMA in CKD: increased methylation of proteins, increased protein turnover, decreased metabolism by DDAH and impaired renal excretion. A strong positive correlation between symmetric dimethyl arginine (SDMA) and creatinine suggests that SDMA might be of value as a marker of the renal function. Reduced NO elaboration secondary to accumulation of ADMA and elevated inflammation may be important pathogenic factors for endothelial dysfunction in patients with the renal disease. Elevation of ADMA may be a missing link between CVD and CKD.

Antibiotic utility and susceptibility changes of multidrug-resistant Escherichia coli and Klebsiella spp: 5-year experience in a tertiary healthcare centre

Summary: The study found that cephalosporins were the most commonly used antibiotics from 2014 to 2018, with an increasing trend in susceptibility of MDR Escherichia coli isolates to ceftriaxone, but a decreasing trend in susceptibility to ciprofloxacin and levofloxacin. Klebsiella spp MDR isolates showed a decreasing trend in susceptibility to cefepime and both fluoroquinolones, with a significant negative association between cefepime consumption and Klebsiella spp MDR isolates susceptibility.

Clinical Presentations, Cluster Analysis and Laboratory-Based Investigation of Aspergillus Otomycosis-A Single Center Experience

Summary: This study investigated the clinical presentation, diagnostic procedure, and relapse in patients with Aspergillus-otomycosis, as well as the biofilm production ability of relapse isolates. The results showed high sensitivity of microscopy and the presence of a specific patient cluster with A. niger complex-otomycosis. Six patients experienced relapse with the same Aspergillus species, all of which had the ability to produce biofilm. Official criteria and recommendations are needed to prevent misdiagnosis and complications.

The Scientific Rationale for the Introduction of Renalase in the Concept of Cardiac Fibrosis

Summary: Cardiac fibrosis is characterized by excessive accumulation of extracellular matrix proteins, leading to ineffective healing and ultimately heart failure. Renalase, a novel protein secreted by the kidneys, shows promise as an anti-fibrotic factor by reducing inflammation, oxidative stress, and fibrotic responses through various mechanisms.

Renalase Challenges the Oxidative Stress and Fibroproliferative Response in COVID-19

Summary: The hallmark of COVID-19 is an inappropriate immune response that leads to inflammation and oxidative stress, resulting in acute respiratory distress syndrome and fibrosis. Renalase, a novel amino-oxidase derived from the kidneys, has shown significant organ protection and potential to alleviate inflammation and fibrosis. Using renalase in experimental models of COVID-19 could be a valuable approach to target acute phase mechanisms and halt fibrosis progression.

Kinetics and mechanism of caffeic acid autoxidation in weakly alkaline aqueous solutions in the presence of Mg(II) ions

Summary: Numerous studies have shown that caffeic acid has various potential uses in medications and cosmetics, as well as therapeutic implications in cardiovascular diseases, various types of cancer, and neurological diseases. This study investigated the kinetics and reaction mechanism of caffeic acid autoxidation in weakly alkaline aqueous solutions in the presence of Mg(II) ions. The results revealed that the presence of Mg(II) ions significantly increased the autoxidation rate and led to the formation of quinones and polymeric products.

First Investigation of the Physiological Distribution of Legacy and Emerging Perfluoroalkyl Substances in Raw Bovine Milk According to the Component Fraction

Summary: This study investigated the distribution of 14 different per-fluoroalkyl substances (PFASs) within milk by analyzing the content in whole milk, skim milk, and cream. Samples were taken from 23 cows in Northern Italy that were not exposed to known sources of PFASs. The most common PFAS found in all fractions was perfluorobutanoic acid (PFBA), followed by perfluorooctane sulfonate (PFOS) and perfluorooctanoic acid (PFOA). PFOS concentration was higher in cream compared to raw and skimmed milk. Multiparous cows had higher levels of PFASs in their milk. Further research is needed to assess the risks of consuming dairy products contaminated with PFASs and their impact on cattle health.

Etiology, Predisposing Factors, Clinical Features and Diagnostic Procedure of Otomycosis: A Literature Review

Summary: Otomycosis is a common fungal infection of the external auditory canal worldwide, with prevalence ranging from 9% to 30%. Aspergillus niger complex and Candida spp. are the main causative agents, with a wide range of other fungi also responsible. The prevalence of otomycosis is highest in Iran, India, China, Egypt, Mexico, and Brazil. Diagnosis is challenging as it can be difficult to distinguish from other infections, but laboratory-based evidence is essential. Treatment involves the use of various topical and systemic antifungal agents.

Umbrella review and meta-analysis of reconstructed individual patient data of mortality following conventional endovascular and open surgical repair of infrarenal abdominal aortic aneurysm

Summary: This umbrella review aimed to assess the quality of published meta-analyses, conduct a de-novo meta-analysis of available RCTs, and test the hypothesis of long-term mortality difference between OSR and EVAR. The results showed that EVAR had significantly favorable mortality up to four years, but there was no difference between EVAR and OSR in the longest available time period. The Cox model demonstrated a non-significant hazard ratio in favor of OSR, while the best-fitting generalized gamma model showed a slightly significant hazard ratio in favor of EVAR.

Efficacy and limitations of ChatGPT as a biostatistical problem-solving tool in medical education: a descriptive study

Summary: The study assessed ChatGPT's (GPT-3.5 and GPT-4) performance as a study tool in solving biostatistical problems and found that both versions had below-average performance. While GPT-3.5 and GPT-4 were able to provide correct answers for many problems, they struggled with certain aspects, particularly ANOVA, chi-square test, and sample size-related questions. Therefore, students need to be aware of the limitations and exercise caution when incorporating this model into medical education.

Potentially inappropriate prescribing among older patients and associated factors: comparison of two versions of STOPP/START criteria

Summary: This study compared the prevalence and types of potentially inappropriate medications (PIMs) and potential prescribing omissions (PPOs) between the original and updated versions of STOPP/START criteria among older patients in different settings. The prevalence of STOPP v2 was slightly higher, but not statistically significant, compared to v1, while the prevalence of START v2 was significantly lower than v1. The most commonly identified PIMs in STOPP v2 were drug indication and fall-risk medications, while cardiovascular system criteria were the most frequently detected in START v2. The number of medications was a strong predictor for both STOPP v1 and v2. The study highlights a substantial rate of potentially inappropriate prescribing among elderly patients.

Joint use of population pharmacokinetics and machine learning for optimizing antiepileptic treatment in pediatric population

Summary: This study used nonlinear mixed-effect modeling to describe the pharmacokinetics of valproic acid (VA), lamotrigine (LTG), and levetiracetam (LEV) in a pediatric population. Machine learning algorithms were applied to identify relationships between plasma levels of the medications and patient characteristics, as well as develop a predictive model for epileptic seizures. The results showed that the kinetics of LTG, LEV, and VA can be best described by a one compartment model with first-order absorption and elimination kinetics. The main factors affecting antiepileptic activity are drug levels and body weight, while gender has no influence. The application of pharmacokinetic and machine learning models can improve epilepsy management during the growth and development of children.
