Summary: This observational study investigated the long-term outcomes and HPV genotype changes after conization for high-grade cervical intraepithelial neoplasia (HG-CIN). Results showed that active surveillance may reduce the severity of progression to cancer, and a majority of patients with recurrent CIN2+ had persistent type-specific HPV infections. Effective therapeutic vaccines for such cases are still needed.

Summary: This study aims to determine the reasonable and optimal conization length for different types of cervical transformation zones (TZs) in order to achieve negative margin outcomes in surgery. The results show that a conization length of 10-15 mm is reasonable for TZ1 and TZ2 patients, while 17-25 mm is optimal for TZ3 patients to obtain more negative internal margins.

Summary: The study found that although there were no significant differences in residual disease and CIN recurrence between the GI-negative and GI-positive groups, GI-positive patients were more likely to receive additional surgical treatments. Careful consideration should be given to repeat surgery based on GI positivity to avoid overtreatment and surgical complications.

Summary: There has been a significant increase in the number of hysterectomies performed for CIN in the past decade. However, this procedure carries risks of complications, the onset of vaginal lesions, and overtreatment, which highlights the importance of proper counseling before making a decision.

Summary: This study assessed the outcomes of a one-time HPV screening in Danish women aged 70+. The study found that the prevalence of high-risk HPV in women aged 70-74 was 4.3%, and there were 5 cases of CIN2+ detected per 1000 screened women, with a recommended treatment threshold of CIN2.

The results revealed that positive ECC, positive margin of LEEP samples, type III transformation zone, and HPV18 infection were reliable prognostic markers of residual disease.

Summary: Objective: To investigate the predictive ability of PAX1 methylation (PAX1(m)) for the pathological upgrade of cervical intraepithelial neoplasia (CIN) before cold knife conization (CKC). Results: PAX1(m) was found to be a potential auxiliary marker in predicting the pathological upgrade of CIN.

Summary: This study investigated the risk factors for pathological upgrading in conization specimens in Chinese women with Cervical Intraepithelial Neoplasia grade 2/3 (CIN2/3) aged 50 and above. Factors associated with upgrading included postmenopausal period >= 5 years, EGI, endocervical curettage, and HPV type 16/18.

Summary: This study investigates the relationship between post-conization HPV status and surgical margins and the absolute risk of cervical intraepithelial neoplasia grade 2 or worse (CIN2+). The results suggest that considering both HPV status and margin in risk assessment provides additional stratification compared to HPV status alone. Combined testing with HPV and margin status has higher sensitivity but lower specificity than HPV testing alone, which is important in high-risk populations.

Positive margins, histories of preterm birth, and complicating diseases were associated with increased risk of recurrence, while cone depth (>0.5 cm) was linked to lower recurrence risk.

Summary: This study found that HR-HPV positivity is a risk factor for recurrence after cervical excision in WLWH, while age older than 41 years is a risk factor for HR-HPV positivity. Therefore, women over 41 may benefit from more frequent follow-up visits. Future studies on HPV vaccination after treatment in WLWH may also be beneficial.

Summary: This retrospective study evaluated the risk factors for recurrent high-grade cervical intraepithelial neoplasia after loop electrosurgical excision procedure (LEEP). High-risk HPV infection, margin status, baseline diagnosis, smoking, and immunosuppression were identified as significant independent risk factors for recurrence. HPV genotypes were associated with higher risk of recurrence.

Summary: This study aimed to investigate guidelines on preterm birth, analyze decision-criteria, and identify consensus and discrepancies among these guidelines. The results showed that a shortened cervix and a history of preterm birth are relevant in singleton pregnancies. Most guidelines recommend progesterone treatment for cases with no history of preterm birth and with a shortened cervix.

Summary: The study found that prior cesarean delivery (CD) in the second stage of labor (non-progressive labor in the second stage - NPL2) is associated with a higher risk of third stage placental complications in the subsequent delivery compared to CD in the first stage of labor (NPL1). Parturients following CD due to NPL2 had significantly higher rates of manual removal of the placenta and adherent placenta in the subsequent delivery. Additionally, adherent placenta was independently associated with vaginal delivery following CD due to NPL2.

Summary: The study showed that patients with advanced ovarian cancer who received dose-dense weekly paclitaxel had significantly better progression-free survival compared to those who received standard 3-weekly chemotherapy. Although the overall survival was also better in the dose-dense group, the difference was not statistically significant. The dose-dense protocol was associated with higher rates of side effects but may be considered as a treatment alternative for advanced ovarian cancer patients.
