Summary: This study investigated umbilical vein blood flow and fetal oxygen and glucose utilization in uncomplicated monochorionic twins, finding that birth order affects umbilical blood flow and birth weight, with second-born twins having lower blood flow and birth weight compared to first-born twins. Additionally, estimated fetal oxygen and glucose consumption were also lower in second-born twins compared to first-born twins.

Summary: This study systematically evaluated the cause of fetal death in a Han Chinese woman with monochorionic twinning following in vitro fertilization/embryo transfer. Discrepant karyotypes were discovered between the twins, with one fetus having an abnormal male karyotype (46, XY) and duplication (9) (p24.3-q13), and the other having a normal female karyotype (46, XX). The male fetus was considered to have died due to chromosome 9 aberration, while the female fetus died due to subsequent acute exsanguination through vascular anastomosis. This study highlights the importance of recognizing monochorionic dizygotic twinning and the challenges of clinical management for twins following IVF/double embryo transfer.

Summary: The study revealed that single intrauterine fetal death may lead to acute feto-fetal transfusion in monochorionic twins, mainly influenced by arterio-arterial anastomoses. The diameter and length of AA anastomoses play a crucial role in transfusion risk, which is related to the distance of umbilical cords insertions.

Summary: This study aimed to evaluate the perinatal outcome of monochorionic twins with single intrauterine fetal death, both spontaneous and after fetal therapy, and to assess the antenatal events that increase the risk of cerebral injury. The study found that the risk of cerebral damage is higher in cases of spontaneous intrauterine fetal death, and gestational age at death, selective intrauterine growth restriction, and anemia of the surviving co-twin are the main predictors for prenatal lesions. The overall perinatal survival rate was 88.3% and abnormal neurological outcomes were related to extreme prematurity.

Summary: In patients with monochorionic diamniotic twins, elective multifetal pregnancy reduction via radiofrequency ablation led to significantly lower rates of preterm birth at <37 weeks and a lower trend of pre term birth at <36 weeks' gestation without an increased risk of pregnancy loss. The median gestational age at delivery was significantly higher in the elective multifetal pregnancy reduction group (38 weeks) than in the ongoing pregnancy group (35.9 weeks). Further research is needed to clarify if multifetal pregnancy reduction improves long-term outcomes.

Summary: In monochorionic twins with selective FGR, neonatal cerebral ultrasound reveals restricted brain growth in the smaller twin, with smaller cerebral structures and less white/deep gray matter. There is a positive linear relationship between birth-weight discordance and intracranial-volume discordance.

Summary: This study found that the earlier onset time of selective fetal growth restriction (sFGR) II and III compared to I may be due to their higher placental territory discordance. The prevalence of thick artery-artery anastomoses is expected to be higher in sFGR III than in I or II.

Summary: A study on congenital heart defects in monochorionic twins showed a six-fold increase compared to singleton or dichorionic twins. Among 23 twin pairs with CHD, 5 were concordant and 18 were discordant. Most cases had discordant phenotypes, suggesting possible environmental and epigenetic influences on the development of discordant CHD in monochorionic twins.

Summary: There is a significant relationship between placental oxygen transport and fetal brain cortical growth and folding in monochorionic twins. Slower placental oxygen delivery is associated with reduced volumetric and surface growth of the cerebral cortex, as well as more divergent sulcal folding patterns.

Summary: Single intrauterine fetal death (sIUFD) in monochorionic diamniotic (MCDA) twin pregnancy is associated with metabolic changes in the surviving co-twin. Metabolomic profiling reveals distinctive metabolic signatures in placental tissue and cord plasma, with some metabolites correlated with fetal growth.

Summary: This retrospective cohort study aimed to identify monochorionic twin pregnancies before blastocyst transfer based on maternal or embryo characteristics registered by time lapse. The study found that inner cell mass grade A and s3 division timing may be associated with an increased risk of monochorionic twinning. Further investigation is needed to determine if these factors reflect a biological background for monochorionic twinning.

Summary: Menstrual symptoms can be barriers to physical activity for women, but hormonal contraceptives may help alleviate these barriers. This study focused on Portuguese athletes' uterine bleeding patterns, contraceptive choices, and the impact of contraception on sports performance. Most athletes reported monthly bleeding patterns, with a majority using contraceptives and only a small percentage believing that contraception may decrease sports performance. Contraceptive users showed lower body mass index, blood loss, and dysmenorrhoea compared to non-users.

Summary: Pregnant kidney transplant recipients are at high risk of complications and require multidisciplinary follow-up. This study found a significant correlation between pre-pregnancy graft function and adverse outcomes, particularly with increased proteinuria and creatinine levels. In patients with these risk factors, stricter monitoring is needed to promptly detect complications.
