Systemic Treatment of Acute Liver Failure with Adipose Derived Stem Cells

Purpose of the study: The definitive treatment for liver failure is, currently, liver transplantation. Research into other possible treatments, focused on achieving regeneration of the liver parenchyma, have led to the development of methods to generate hepatocytes from stem cells. In our study, we transplant allogenic adipose-derived stem cells (ASCs), not previously differentiated to hepatocytes, to treat acute liver failure induced by intraperitoneal administration of carbon tetrachloride (CCl4) in a Sprague-Dawley rat model. Material and Methods: The ASCs were delivered via the tail vein, having previously been labeled with PKH26, a fluorescent membrane marker. Two control groups were established, Group 1(n = 15) consisting of olive oil (5 mL/kg) and Group 2(n = 15): 1 x 10(6) PKH26-labeled ASCs. Further, two study groups, Group 3(n = 30): CCl4 dissolved in olive oil and Group 4(n = 30): CCl4 dissolved in olive oil and 1 x 10(6) PKH26-labeled ASCs completed the experimental design. Results: Blood samples were analyzed, finding AST and ALT levels significantly higher in treatment over control groups at 24 and 48 hours. The mortality rates were statistically different between control groups and Group 3 (Group 1-3 p = .04, Group 2-3 p = .04) and between Groups 3 and 4 (p = .02). Examining the liver parenchyma, a significantly higher number of ASCs were observed in Group 4 than in Group 2 at all time points (p = .00). Conclusions: The intravenous injection of allogenic ASCs in this model of CCl4-induced liver failure reduced the mortality in treated animals. ASCs injected in the rat tail vein were found in the liver in animals with induced acute liver failure.

Summary: Variation in the uptake of ERAS principles was observed across an international network of colorectal units, with some principles being universally adhered to while others showed inconsistency. Adherence to principles related to prehabilitation, managing anaemia, and postoperative nutrition appeared to have increased since 2017 according to the survey results. Further research into specific principles is needed to improve the implementation of ERAS guidelines.

Summary: This multicenter retrospective study evaluated the short and long-term outcomes of surgical treatment for renal cell carcinoma (RCC) pancreatic metastases. The results showed that pancreatic resection improved overall survival and disease-free survival in these patients, even if extrapancreatic metastases had been previously removed. Total pancreatectomy is not always necessary in cases of intrapancreatic multifocal compromise to improve survival.

Summary: In the era of personalized medicine and targeted therapies for cancer patients, ultrasensitive detection methods like ddPCR play a significant role in tumor genotyping. Liquid biopsy using ddPCR has seen a substantial increase in publications, providing reliable tools for detecting nucleic acid-based markers and monitoring disease progression in various cancers. Additionally, ddPCR is being investigated in a wide range of biologic fluids as alternative sources for genetic material in cancer profiling.

Summary: This study provides a global overview of the management of patients with acute cholecystitis during the early phase of the COVID-19 pandemic. The pandemic led to a decrease in surgical workforce and operating facilities, worse severity of disease, and increased use of conservative management. While the number of patients undergoing cholecystectomy decreased, there was no increase in post-operative mortality.

Abdominal Organ Preservation Solutions in the Age of Machine Perfusion

Summary: The past decade has witnessed a revolution in the field of abdominal organ transplantation, with perfusion increasingly replacing static cold storage as the preferred preservation method. Perfusion offers advantages such as continuous supply of metabolic substrates, waste removal, and organ viability assessment. However, static cold storage still remains important for logistical convenience and cost-effectiveness. This review provides an overview of organ preservation concepts, the history of static cold preservation, different preservation solutions, and upcoming innovations in preservation solutions.

Deep Learning to Measure the Intensity of Indocyanine Green in Endometriosis Surgeries with Intestinal Resection

Summary: This study proposes a novel method based on deep learning algorithms for quantifying the level of blood perfusion in anastomosis. The results show that segmentation of the bowel using the U-Net was successful, and textures are appropriate descriptors for characterization of blood perfusion in images where ICG is present. The findings of this study may help predict the occurrence of postoperative complications during surgery and guide clinicians in taking appropriate actions.

Quality of life and low anterior resection syndrome before and after deep endometriosis surgery

Summary: The study aimed to evaluate gastrointestinal functional outcomes and quality of life in patients who underwent surgery for deep endometriosis (DE). The results showed that rectal surgery improved the quality of life of DE patients and did not negatively affect the occurrence of low anterior resection-like syndrome, regardless of the surgical technique performed.
