Background. Persistent human papillomavirus (HPV) infection is the most important risk factor for cervical cancer, and understanding genotype-specific HPV persistence is essential for elucidating the natural history of HPV infections. Methods. In the Finnish Family HPV Study, 329 pregnant women (mean age, 25.5 years) were recruited during the third trimester of pregnancy and were followed up for 6 years. Multiplex HPV genotyping for 27 low-and high-risk HPV types was used to define genotype-specific prevalence at each visit. Generalized estimating equation models were constructed to estimate predictors of type-specific persistence (positive results at 2 consecutive visits) of species 7 and 9 HPV genotypes. Results. HPV16 was the most common type, followed by HPV types 18, 31, 35, 45, 58, 70, and 6. Prevalence of multiple infections ranged from 21% to 45%. Persistence was most prolonged for HPV types 35, 58, and 52, with durations of 38.7, 32.1, and 24.2 months, respectively, and was equal for multiple-type infections and HPV16, with durations of 21 and 24 months, respectively. Independent predictors of type-specific persistence of species 7 and 9 HPV genotypes were age (odds ratio, 1.13 [95% confidence interval, 1.02-1.25]; P=.017), oral sex (odds ratio, 0.37 [95% confidence interval, 0.17-0.81]; P=.013), and young age (! 13 years) at initiation of smoking (odds ratio, 0.51 [95% confidence interval, 0.27-0.98]; P=.046). Conclusion. HPV16 was the most frequent persisting HPV genotype followed by multiple infections. Early initiation of smoking, practicing oral sex and older age increase the risk for persistence of species 7 and 9 HPV genotypes.

Assessing the risk of cervical neoplasia in the post-HPV vaccination era

Matti Lehtinen, Ville N. Pimenoff, Belinda Nedjai, Karolina Louvanto, Lisanne Verhoef, Danielle A. M. Heideman, Mariam El-Zein, Martin Widschwendter, Joakim Dillner

Summary: This article is based on a recent scientific session that discussed the risk assessment of cervical cancer in the post-vaccination era. The study highlighted the impact of decreasing prevalence of vaccine-targeted high-risk HPV types on the triage methods for cervical intraepithelial neoplasia (CIN)/squamous intraepithelial lesion (SIL). The research emphasized the importance of evaluating new triage modalities in vaccinated women with CIN/SIL associated with nonvaccine HPV types.

A synthesis of evidence for cancer-specific screening interventions: A Preventive Medicine Golden Jubilee Review

Suvi Rintala, Kristina R. Dahlstrom, Eduardo L. Franco, Karolina Louvanto

Summary: The goal of cancer screening guidelines is to provide evidence-based recommendations for cancer prevention. Various organizations have conflicting recommendations, but current guidelines suggest colorectal cancer screening with fecal occult blood test or fecal immunochemical test, cervical cancer screening with Pap-test, HPV-test, or co-testing, and breast cancer screening with mammography. Guidelines differ in terms of age to start and end screening and testing frequency. Constant review of screening guidelines is important to evaluate new screening tests.

Seroprevalence of polyomaviruses BK and JC in Finnish women and their spouses followed-up for three years

Hanna K. Laine, Tim Waterboer, Kari Syrjanen, Seija Grenman, Karolina Louvanto, Stina Syrjanen

Summary: This study analyzed the seroprevalence and antibody levels of BKPyV and JCPyV in 327 pregnant women and their 132 spouses in Finland. The results showed that BKPyV and JCPyV infections were common in married couples, with slightly higher prevalence in women and stable antibody levels.

Human polyomavirus BKPyV and JCPyV serostatus has no impact on women′s human papillomavirus infection outcome

Summary: This study aimed to investigate the association between BK (BKPyV) and/or JC (JCPyV) polyomavirus serology and HPV in Finnish women. The results showed no significant association between BKPyV or JCPyV seropositivity and HPV seropositivity, HPV DNA positivity, HPV16 persistence, Pap smear grade, or development of CIN.

Retrospective Analysis of Artifacts in Cone Beam Computed Tomography Images Used to Diagnose Chronic Rhinosinusitis

Summary: The study assessed the frequency and location of artifacts in cone beam computed tomography (CBCT) images used to confirm chronic rhinosinusitis (CRS). It was found that large dental fillings or crowns, as well as endodontic fillings, were the main origins of the artifacts. The presence of artifacts might lead to difficulties in diagnosing apical periodontitis (AP) and could also affect the diagnosis of CRS.

Human Papillomavirus Concordance Between Parents and Their Newborn Offspring: Results From the Finnish Family Human Papillomavirus Study

Nelli T. Suominen, Tiina H. Luukkaala, Claudie Laprise, Marjut A. Haataja, Seija E. Grenman, Stina M. Syrjaenen, Karolina Louvanto

Summary: This study aimed to determine whether vertical human papillomavirus (HPV) transmission occurs from parents to their offspring before or during birth. The study included 321 mothers, 134 fathers, and their 321 newborn offspring. The results showed statistically significant genotype-specific HPV concordance between mother-newborn and father-newborn pairs, suggestive for vertical HPV transmission, but transmission from the father to the newborn remains uncertain.

Summary: This study found that a second pregnancy does not have a significant impact on HPV antibody levels, but it may influence serological outcomes, especially in ongoing second pregnancies.

Added value of electrical impedance spectroscopy in adjunction of colposcopy: a prospective cohort study

Summary: This study aimed to evaluate whether electrical impedance spectroscopy (EIS) as an adjunctive technology improves the performance of colposcopy. The results showed that colposcopy with EIS had higher sensitivity but lower specificity to detect cervical intraepithelial neoplasia grade 2 or worse (CIN2+). However, the prevalence rates of CIN2+ were not consistently higher in the EIS cohort, indicating the need for further research on the effectiveness of EIS.

Summary: This study reports a severe case of oligodontia in a female patient with the EDA gene variant c.742-2A>G. The results strongly suggest that this variant is pathogenic and the oligodontia in the patient is exceptionally severe. Early identification of this variant is crucial for planning adequate treatment and follow-up.

HLA-G Alleles Impact the Perinatal Father-Child HPV Transmission

Summary: This study evaluated the importance of human leukocyte antigen (HLA)-G alleles in father-to-child human papillomavirus (HPV) transmission during the perinatal period. The results showed that HLA-G allele concordance was associated with the father's urethral and child's oral high-risk HPV concordance at birth. In addition, HLA-G allele concordance increased the risk of father's oral and child's postpartum oral any- and high-risk HPV concordance.

Ecological diversity profiles of non-vaccine-targeted HPVs after gender-based community vaccination efforts

Ville N. Pimenoff, Penelope Gray, Karolina Louvanto, Tiina Eriksson, Camilla Lagheden, Anna Soderlund-Strand, Joakim Dillner, Matti Lehtinen

Summary: The long-term effect of HPV vaccination on non-targeted HPVs is not well understood. An 8-year follow-up study found that gender-neutral vaccination communities showed increased HPV diversity 4 to 8 years post-vaccination, despite the clearance of targeted HPVs. This may impact future cervical cancer screening programs but not the WHO's mission to eliminate cervical cancer.
