Thrombocytopenia and anemia caused by a persistent high linezolid concentration in patients with renal dysfunction

It has been proposed that it is not necessary to adjust the dose of linezolid (LZD) in patients with reduced renal function. However, significantly lower platelet counts and hemoglobin levels have been reported in such patients compared to those in patients with normal renal function. This suggests that the appropriate dose and administration method for LZD are yet to be established in patients with renal dysfunction. The subjects in this study were patients with renal dysfunction who developed adverse effects of thrombocytopenia and anemia during treatment with LZD. We investigated the association of these adverse effects with the blood LZD concentration and the area under the concentration-time curve from zero to 24 h (AUC(0-24)), determined using a one-compartment Bayesian model (n = 20). The measured blood LZD concentration was significantly higher than the predicted concentration in a population pharmacokinetics approach (p < 0.01), and severe thrombocytopenia developed as the blood LZD concentration increased. The platelet count and hemoglobin level decreased as the AUC(0-24) of LZD increased in patients with renal dysfunction, and the correlations were significant: r = 0.593 and r = 0.783, respectively (p < 0.01). These findings suggest that LZD administered to patients with renal dysfunction may reach a high blood level and subsequently increase the AUC(0-24), which may then induce adverse effects of severe thrombocytopenia and anemia.

Evaluation of the relationship between linezolid exposure and hyponatremia

Proposal of initial and maintenance dosing regimens with linezolid for renal impairment patients

Based on the findings, it is recommended to consider empirical dose reduction to 300 mg every 12 h after the initial fixed dose for 2 days and the use of TDM in patients with renal impairment to improve safety outcomes.

Computational design of clinical trials using a combination of simulation and the genetic algorithm

Summary: Artificial intelligence (AI) is being used in various technological fields, but there have been no reports of AI-designed clinical trials. In this study, a genetic algorithm (GA) was used to develop study designs, applying computational design approach to optimize blood sampling schedule for a pediatric bioequivalence study and allocation of dose groups for a dose-finding study. The GA reduced the number of blood collection points and achieved a reduction in the required subjects for the dose-finding study. The results showed the potential usefulness of computational clinical study design approach in innovative drug development.

External Evaluation of a Bayesian Warfarin Dose Optimization Based on a Kinetic-Pharmacodynamic Model

Summary: This study evaluated the predictive performance of a kinetic-pharmacodynamic model for warfarin dose requirements in Japanese patients. The study found that the model tended to underpredict, especially in patients over 60 years old, with a body mass index of 25 kg/m(2) or lower, and an estimated glomerular filtration rate of 60 mL/min/1.73 m(2) or lower. However, underprediction was reduced by using Bayesian prediction and more previous observations.

Efficient implementation of hepatitis B surface antigen confirmatory neutralization tests

Summary: The aim of this study was to identify the risk factors for false positives in high-sensitivity HBsAg quantitative tests and reduce the need for neutralization tests. The results showed that younger age, female sex, lower HBsAg values, and reagent improvement were independent risk factors for false positives. The false-positive rate was highest in the range of 0.005-0.049 IU/mL.
