beta-D-Glucan and S-adenosylmethionine serum levels for the diagnosis of Pneumocystis pneumonia in HIV-negative Patients: A prospective study

Objective: To prospectively assess the diagnostic utility of S-adenosylmethionine (AdoMet) and (1 -> 3)-beta-D-glucan (beta-D-glucan) serum markers for Pneumocystis pneumonia (PCP) in HIV-negative patients. Methods: HIV-negative, immunocompromised patients suspected of PCP based on clinical presentation and chest imaging were included. PCP was confirmed or rejected by results of direct microscopy and/or real-time PCR on broncho-alveolar lavage (BAL) fluid. Measurement of serum beta-D-glucan and AdoMet was performed on serum samples collected at enrollment and during follow-up. Both serum beta-D-glucan and AdoMet were assessed for diagnostic accuracy and correlation with clinical and laboratory parameters. Results: In 31 patients enrolled (21 PCP-positive, 10 PCP-negative), AdoMet levels did not discriminate between patients with and without PCP. Elevated serum beta-D-glucan was a reliable indicator for PCP with a sensitivity of 0.90 and specificity of 0.89 at the 60 pg/ml cut-off. In PCP-positive patients beta-D-glucan serum levels decreased during treatment and inversely correlated with Pneumocystis PCR cycle threshold values in BAL fluid. Conclusions: The level of serum beta-D-glucan-but not AdoMet - was diagnostic for PCP within the clinical context and may serve as marker for pulmonary fungal load and treatment monitoring. Summary: This study compared the performance of Wako and Fungitell beta-D-glucan assays in patients with eumycetoma. The results showed that when using the optimized cut-off value, the Wako assay and Fungitell assay had comparable performance in terms of sensitivity and specificity. Therefore, the Wako assay can be used in combination with current diagnostic tools.

Comparing the performance of the common used eumycetoma diagnostic tests

Summary: This study compared the diagnostic performance of ultrasound, histopathology, culture, and two species-specific PCRs in identifying the causative organism of mycetoma. The results showed that the two PCR methods were the most sensitive and specific, while ultrasound was the least specific. The evaluation concluded that PCR directly on DNA isolated from grains is the most rapid and reliable diagnostic tool.

Effect of invasive aspergillosis on risk for different causes of death in older patients with acute myeloid leukaemia or high-risk myelodysplastic syndrome

Summary: The study aimed to estimate the cumulative incidence of death due to different causes of death (CODs) in older patients with AML or high-risk MDS, and investigate the effect of IA on each separate COD. The results showed that infection was the most common cause of death in the first year after diagnosis, while leukemia became the most likely cause of death over time. Patients with IA had an increased risk of dying from leukemia or infection.

Antifungal Activity of Natural Naphthoquinones and Anthraquinones against Madurella mycetomatis

Summary: In this study, a series of natural naphthoquinones and anthraquinones were evaluated for their activity against the most common causative agent of eumycetoma. Naphthazarin was identified as a considerably active antifungal compound against Madurella mycetomatis, with reduced toxicity to Galleria mellonella larvae.

Evaluation of the Antiparasitic and Antifungal Activities of Synthetic Piperlongumine-Type Cinnamide Derivatives: Booster Effect by Halogen Substituents

Summary: Synthetic derivatives of the natural alkaloid piperlongumine, N-acylpyrrolidones and -piperidones, were prepared and tested against Leishmania major and Toxoplasma gondii parasites. Replacement of an aryl meta-methoxy group with halogens increased antiparasitic activities. Some compounds showed strong activity against L. major promastigotes and high activity against T. gondii parasites. QSAR studies and docking calculations revealed binding differences between 2-pyrrolidone and 2-piperidone derivatives. Compound 4 b exhibited microtubules-destabilizing effects in T. b. brucei cells.

Trends in antibiotic selection pressure generated in primary care and their association with sentinel antimicrobial resistance patterns in Europe

Summary: This study investigated trends in antibiotic prescribing in primary care and examined the associations between antibiotic selection pressure (ASP) and the prevalence of drug-resistant microorganisms. The findings revealed significant associations between the volume of antibiotic prescribing, ASP, and the prevalence of drug-resistant microorganisms. Countries with higher antibiotic volumes and ASP had higher prevalence of drug-resistant microorganisms. The study suggests that the impact of ASP generated from primary care on increasing antimicrobial resistance may be larger than currently assumed.

Summary: The Dutch Working Party on Antibiotic Policy (SWAB) established a multidisciplinary expert committee to provide evidence-based recommendations for antibiotic therapy in patients with reported antibiotic allergies. A total of 60 recommendations were generated for suspected allergy to β-lactam antibiotics (BLAs) and non-β-lactam antibiotics, based on systematic literature searches and evaluations.

A Falciformispora senegalensis grain model in Galleria mellonella larvae

Summary: Eumycetoma is a common subcutaneous implantation mycosis with the formation of black or white grains. In this study, a grain model of F. senegalensis was established in G. mellonella larvae, and the efficacy of antifungal therapy was evaluated. The results showed that amphotericin B can prolong larval survival. This model can be used to monitor grain formation and study the effectiveness of antifungal drugs.

The First Case of Fusarium falciforme Eumycetoma in Sudan and an Extensive Literature Review about Treatment Worldwide

Summary: This is a case report on a fungal infection caused by Fusarium falciforme, resulting in a recurrent eumycetoma. This is the first report of Fusarium falciforme causing eumycetoma in Sudan, highlighting the need for increased awareness among healthcare providers and improved diagnostic and surveillance systems in at-risk areas to improve case management and reduce the threat of further spread.

Summary: This systematic review and meta-analysis evaluated the outcome of DAIR in treating streptococcal hip and knee prosthetic joint infection (PJI). The study included 25 observational studies with a total of 1367 patients. The overall success rate of streptococcal PJI treated with DAIR was 71%, with knee PJI showing a higher success rate (76%) compared to hip PJI (58%).
