Local Delivery of GM-CSF Protects Mice from Lethal Pneumococcal Pneumonia

The growth factor GM-CSF has an important role in pulmonary surfactant metabolism and the regulation of antibacterial activities of lung sentinel cells. However, the potential of intra-alveolar GM-CSF to augment lung protective immunity against inhaled bacterial pathogens has not been defined in preclinical infection models. We hypothesized that transient overexpression of GM-CSF in the lungs of mice by adenoviral gene transfer (Ad-GM-CSF) would protect mice from subsequent lethal pneumococcal pneumonia. Our data show that intra-alveolar delivery of Ad-GM-CSF led to sustained increased pSTAT5 expression and PU.1 protein expression in alveolar macrophages during a 28-d observation period. Pulmonary Ad-GM-CSF delivery 2-4 wk prior to infection of mice with Streptococcus pneumoniae significantly reduced mortality rates relative to control vector-treated mice. This increased survival was accompanied by increased inducible NO synthase expression, antibacterial activity, and a significant reduction in caspase-3-dependent apoptosis and secondary necrosis of lung sentinel cells. Importantly, therapeutic treatment of mice with rGM-CSF improved lung protective immunity and accelerated bacterial clearance after pneumococcal challenge. We conclude that prophylactic delivery of GM-CSF triggers long-lasting immunostimulatory effects in the lung in vivo and rescues mice from lethal pneumococcal pneumonia by improving antibacterial immunity. These data support use of novel antibiotic-independent immunostimulatory therapies to protect patients against bacterial pneumonias. The Journal of Immunology, 2011, 187: 5346-5356.

Connective-Tissue Growth Factor Contributes to TGF-β1-induced Lung Fibrosis

Summary: Idiopathic pulmonary fibrosis is a fatal lung disease characterized by the accumulation of myofibroblasts. The study found that connective-tissue growth factor (CTGF) plays a key role in pulmonary fibrosis. Increased levels of CTGF exacerbate lung fibrosis and CTGF expression is upregulated in patients with idiopathic pulmonary fibrosis. Different cells contribute to the maintenance of the fibrotic phenotype during fibrogenesis.

Summary: This study investigated the role of vascular remodeling in fibrotic lungs and its contribution to the progression of fibrosis. It was found that endothelial dysfunction and activation of vascular smooth muscle cells enhance fibrogenesis through impaired BMPR2 signaling. Tacrolimus may have therapeutic potential in treating advanced IPF and concomitant PH. Additionally, genetic abnormalities may determine the development of PH in advanced IPF.

Summary: Type 1 alveolar epithelial cells (AT1s) and type 2 alveolar epithelial cells (AT2s) regulate the structural integrity and function of alveoli. Gremlin, an inhibitor of bone morphogenetic protein (BMP), induces AT2 proliferation and differentiation. It was found that Gremlin expression during lung injury leads to inhibition of BMP signaling and activation of AT2s, promoting alveolar repair.

Therapeutic strategies targeting pro-fibrotic macrophages in interstitial lung disease

Summary: Idiopathic pulmonary fibrosis (IPF) is a type of interstitial lung disease characterized by severe scarring in the lung interstitium. Macrophages play a crucial role in the development of pulmonary fibrosis, and recent studies have revealed the heterogeneity and plasticity of pro-fibrotic macrophages. This review discusses the role and characterization of pro-fibrotic macrophages in fibrotic lung diseases and explores the potential of targeting macrophage polarity as a therapeutic strategy for lung fibrosis.

Summary: Idiopathic pulmonary fibrosis (IPF) is a chronic, progressive lung disease with unknown cause, characterized by fibrosis. Macrophage accumulation is involved in the pathogenesis of IPF, and the unfolded protein response (UPR) is linked to macrophage activation. The impact of ATF6a, a mediator of UPR, on pulmonary macrophage composition and function in lung injury and fibrogenesis is unclear. This study found that Atf6a is expressed in pro-fibrotic macrophages in lung tissues of IPF patients and circulating monocytes. Deletion of Atf6a in myeloid cells altered macrophage composition and exacerbates fibrogenesis. Further investigation revealed that ATF6a is required for CHOP induction and macrophage death. These findings indicate a detrimental role for ATF6a-deficient macrophages in lung injury and fibrosis.

Oncolytic BHV-1 Is Sufficient to Induce Immunogenic Cell Death and Synergizes with Low-Dose Chemotherapy to Dampen Immunosuppressive T Regulatory Cells

Summary: Immunotherapy stimulates the patient's immune system to fight cancer, but some tumors are immune cold and do not respond well to immune checkpoint therapies. Oncolytic viruses preferentially target tumor cells and can stimulate immune responses. An oncolytic virus based on a common human herpesvirus has been approved by the FDA and used in cancer immunotherapy. In this study, an oncolytic bovine herpesvirus is shown to activate immune responses in a mouse model, and the addition of low-dose chemotherapy enhances immune responses and enables immune checkpoint therapy to clear tumors.
