Newly developed renin and prorenin assays and the clinical evaluation of renin inhibitors

Background The last decade has seen the introduction of renin inhibitors and new plasma renin and prorenin assays, which has led to a better understanding of the tissue renin - angiotensin system. Aim of the study To clarify the consequences of these developments for the methodology and interpretation of measurements of renin and prorenin. Methods The principles and application of the newly developed immunosorbent assays (ISAs) are surveyed and the results are compared with those of enzyme-kinetic assays (EKAs). Results and conclusions Angiotensin (Ang) II in cardiac, renal and adrenal tissue is known to originate mainly from locally produced Ang I. Experimental evidence and theoretical considerations show that a simple relation between Ang II receptor occupancy, in tissue micromilieu, and the circulating levels of Ang II or renin may not exist. This supports the clinicians' view that the plasma level of renin tells more about the mechanisms regulating its release into the circulation than about the Ang II-dependency of hypertension. ISAs are a welcome addition to clinical studies of renin inhibitors. By comparing the results of ISAs with those of EKAs, the inhibitor-bound forms of renin and prorenin can be distinguished from the unbound forms. ISAs also provide important information on the molecular basis of prorenin activation. We propose a single kinetic model to incorporate the conformational changes of prorenin induced by cryo-activation and acid-activation, and by binding to renin inhibitors. It explains why renin ISAs can overestimate the rise of renin in response to these drugs, and shows how to deal with this artefact.

Summary: AVS is the standard lateralization technique in PA. International data showed lower rates of unilateral PA and adrenalectomy in Japanese centers compared to European centers. Reasons for not performing adrenalectomy in patients with unilateral PA were more likely to be physician-derived in Japan and patient-derived in Europe.

Functional tests to guide management in an adult with loss of function of type-1 angiotensin II receptor

Summary: This study reports a case of a 28-year-old male with a homozygous truncating mutation in AGTR1 gene, demonstrating that even truncating loss-of-function mutations in AGTR1 are compatible with life and relatively good GFR. The patient-tailored functional test showed that high doses of fludrocortisone induced renal potassium excretion and mitigated salt-wasting, providing evidence for the prescription of fludrocortisone in such patients.

Summary: This study aimed to determine the rate of drug-resistant arterial hypertension in patients with primary aldosteronism (PA) and investigate the diagnostic performance of adrenal vein sampling (AVS). The results showed that resistant hypertension was common in PA patients, and AVS-guided adrenalectomy effectively resolved treatment-resistant hypertension.

Summary: This study aimed to establish signatures for different forms of endocrine hypertension (EHT) and investigate unbiased disease biomarkers. Plasma samples from 13 biobanks across seven countries were analyzed using untargeted NMR metabolomics. While signatures that could differentiate different forms of EHT were found, confounding effects related to sample center and age were also identified. Various approaches were applied to correct these confounding effects, but no biomarkers for differentiating primary hypertension from EHT could be identified.

Summary: Hypertension is a major global health problem with high prevalence and complex associated health risks. Primary hypertension is the most common form, while the reasons behind it are largely unknown. Endocrine hypertension is another complex form often misdiagnosed as primary hypertension. This study used targeted metabolomics and machine learning methods to predict key biomarkers and successfully classify different subtypes of hypertension.
