Hospitals During Recession and Recovery: Vulnerable Institutions and Quality at Risk

For generations, American hospitals have been considered recession-proof, but there is reason to believe the current economic crisis is an exception. Hospitals have shown declining financial margins and decreased admissions. The severe recession has adversely affected many hospitals' finances, creating a risk of closure and constraining plans for expansion. We believe there is also a risk of harming clinical quality, through decreased staffing that may limit the momentum of the hospital quality movement, especially in fiscally vulnerable institutions. We consider ways the federal government could aid hospitals by promoting hospital quality while providing employment. Journal of Hospital Medicine 2010;5:302-305. Monitoring policy in the context of preventive treatment of cardiovascular disease

Summary: Preventing chronic diseases is crucial in medical care. Physicians prevent chronic diseases by monitoring risk factors and prescribing necessary medication. The optimal monitoring policy depends on the patient's risk factors and demographics. This study proposes a finite horizon and finite-state Markov decision process to define monitoring policies, using stochastic models based on longitudinal observational data from electronic health records. The research investigates policies for cholesterol-lowering medication and the influence of gender and race on optimal monitoring policies.

Nationwide Implementation of a Population Management Dashboard for Monitoring Direct Oral Anticoagulants: Insights From the Veterans Affairs Health System

Summary: A population management dashboard was developed and implemented in the Veterans Health Affairs system to support pharmacist review of anticoagulant prescribing. Between August 2016 and June 2020, 150 sites used the dashboard, with an average of 1875 patients per site. The dashboard demonstrated rapid and sustained implementation and adoption.

Association Between Acute Myocardial Infarction and Cognition

Summary: This cohort study using pooled data from 6 cohort studies conducted from 1971 to 2019 found that incident myocardial infarction (MI) was not associated with a decrease in global cognition, memory, or executive function at the time of the event compared with no MI, but was associated with faster declines in these cognitive functions over time. These findings suggest that prevention of MI may be important for long-term brain health.

This study found that higher poststroke glucose levels were associated with faster global cognitive decline, but poststroke LDL cholesterol and SBP levels did not show any association with cognitive decline.

Summary: This study examined the impact of individual clinician authority on the implementation success of an EHR DOAC Dashboard for safe medication prescribing. The results showed that a high level of individual clinician authority was associated with key facilitators for effective use of the DOAC Dashboard, while a lack of individual authority was often associated with barriers to effective use.
