Role of contaminated aspiration tubes in nosocomial outbreak of Klebsiella pneumoniae producing SHV-2 and CTX-M-15 extended-spectrum β-lactamases

摘要

Klebsiella pneumoniae resistant to ceftazidime was isolated from ten neonates hospitalised between February and March 2006 in two Antananarivo hospitals, Madagascar. The main environmental source, for one hospital in particular, was the liquid used to rinse aspiration tubes in the paediatric wards. The risk of contamination from aspiration tubes is very high in the hospitals of Antananarivo since tap water used to rinse the tubes is not regularly changed. Phenotypical (biotyping and antibiotyping) and genotypical (pulsed-field get electrophoresis) analysis of all the clinical isolates indicated that nine cases were due to a single clone. This clone carried the genes encoding SHV-2 and CTX-M-15 beta-tactamases. This is the first description of an epidemic due to an ESBL-producing member of the family Enterobacteriaceae in Malagasy hospitals. (C) 2009 The Hospital Infection Society. Summary: Understanding cefiderocol resistance mechanisms is crucial for developing solutions against infections and resistance development. Cefiderocol resistance in Klebsiella pneumoniae AR 0047 is associated with SHV beta-lactamase activities and loss of the outer membrane porin. The combination of cefiderocol and avibactam presents an opportunity to enhance effectiveness against cefiderocol-resistant pathogens.

Summary: The prevalence of carbapenem-resistant Klebsiella pneumoniae is increasing, with reports of non-carbapenemase-producing K. pneumoniae showing high-level resistance to carbapenems. In this study, a carbapenemase-negative carbapenem-resistant K. pneumoniae was isolated from a patient, and resistance genes and plasmid transfer were detected. This study reveals the resistance mechanism of this strain.

Summary: This study investigated the frequency, characteristics and antibiotic resistance of nosocomial extended spectrum beta lactamase producing Klebsiella species. The results showed that these Klebsiella pneumoniae strains exhibited resistance to multiple antibiotics and had the ability to form biofilm and produce virulence genes. These resistant strains pose a public threat to hospitalized patients.

Is Carbapenem Therapy Necessary for the Treatment of Non-CTX-M Extended-Spectrum β-Lactamase-Producing Enterobacterales Bloodstream Infections?

Summary: This study investigated the clinical outcomes and optimal treatment methods for patients with ESBL-E bloodstream infections that do not produce CTX-M. The findings suggest that non-CTX-M and CTX-M ESBL-E infections are equally concerning and associated with similar clinical outcomes. Meropenem may be associated with improved survival in patients with non-CTX-M ESBL-E infections, highlighting the potential benefit of comprehensive molecular diagnostics for early antibiotic optimization in ESBL-E infections.

Summary: CTX-M-2-producing Klebsiella oxytoca strains with high ESBL production rates may spread clonally within a farm, while the bla(CTX-M-2) gene in Klebsiella pneumoniae may disseminate among farms through different plasmids. Monitoring ESBL genes, including bla(CTX-M-2), among causative agents of bacterial mastitis in cows can help develop relevant treatments and control practices.

Summary: Globally, antibiotic-resistant Klebsiella spp. cause healthcare-associated infections with high mortality rates. The study investigated the phenotypic and genetic features of Klebsiella isolates from invasive infections in Mozambique. The majority of isolates displayed multidrug resistance, with ESBL-producing strains carrying various beta-lactamase genes. The study also identified susceptible hypervirulent Klebsiella pneumoniae isolates associated with severe infections. The findings highlight the importance of surveillance strategies to prevent the emergence of multidrug-resistant hypervirulent Klebsiella pneumoniae.
