Investigation of an outbreak of Enterobacter cloacae in a neonatal unit and review of the literature

Enterobacter cloacae has emerged as an important pathogen in neonatal units, with several outbreaks of infection being reported. The aim of this study was to investigate an outbreak of sepsis due to E. cloacae in a neonatal unit and to review the literature. A retrospective cohort study was conducted in which cases were compared with all newborns hospitalised for more than 48 h in the neonatal intensive care unit (NICU). Cohorting of infected patients and work reorganisation were implemented. Pulsed-field get electrophoresis was performed. The retrospective cohort included the six cases and 13 control patients that had been in the NICU during April 2006. Univariate analysis showed that the use of dobutamine was significantly associated with infection (P=0.036) and that enteral feeding was a protective factor (P = 0.02). Multivariate analysis did not find any independent risk factor. Bed occupancy rate in March 2006 was 109.6%, indicating overcrowding. PFGE identified indistinguishable patterns among isolates from all six newborns. PubMed and OVID was search from 1 January 1983 to 15 January 2008 for papers including the terms 'E. cloacae', 'out-breaks', 'clusters' in combination with 'neonate', 'newborn', and 'infant'. We found 26 reports of outbreaks due to E. cloacae in neonate patients: sixteen (52%) were bloodstream infection outbreaks, of which two (12.5%) were related to multiple-dose medications. The source for our outbreak was not identified. Reinforcement of hygiene practices, restrictions on new admissions and the establishment of single-dose medications helped to control the outbreak. This study aims to evaluate the relationship between chemosensory dysfunction and neuropsychiatric impairments in a well-characterized sample of post-COVID-19 patients. The findings indicate a positive association between chemosensory abnormalities and cognitive dysfunction in these patients.

Summary: This study presents a rare case of gonococcal arthritis in a patient with Systemic Lupus Erythematosus (SLE). Whole genome sequencing (WGS) and molecular analysis were performed on the etiologic agent, which was found to be resistant to ciprofloxacin and tetracycline. The isolate had genetic features related to beta-lactam, tetracycline, and quinolone resistance, and 71 virulence genes were identified.

Summary: This study evaluated the prevalence of RT-PCR positivity in asymptomatic healthcare workers (HCW) and its impact on absenteeism. A significant decrease in absenteeism was detected 3-4 weeks after the intervention. The testing and leave policy for asymptomatic professionals contributed to the control strategy for COVID-19 transmission in the hospital environment.

Reinfection rate in a cohort of healthcare workers over 2 years of the COVID-19 pandemic

Summary: In this study, the rate of reinfection by SARS-CoV-2 over a two-year period of the COVID-19 pandemic was estimated in a large cohort of healthcare workers. It was found that approximately 5% (284 cases) of symptomatic cases experienced reinfection, with the majority occurring during the Omicron variant period. All cases of reinfection were mild.

Chlorhexidine susceptibility and Eagle effect in planktonic cells and biofilm of nosocomial isolates

Summary: The aim of this study was to evaluate the susceptibility of 32 gram-negative and 6 gram-positive isolates to chlorhexidine gluconate (CHG) in both planktonic cells and biofilm. The study found that gram-negative isolates had higher minimal concentrations than gram-positive isolates, and bacteria in biofilms were more tolerant than planktonic ones. There was no statistically significant correlation between minimal bactericidal concentration or minimal biofilm elimination concentration and biofilm formation. The occurrence of the Eagle effect was observed in 60.5% of the isolates, resulting in minimal concentration of CHG as high as 16,384 mu g/mL. Attention should be given to this phenomenon to avoid false susceptibility results.

COVID-19 in hematopoietic stem-cell transplant recipients: A systematic review and meta-analysis of clinical characteristics and outcomes

Summary: This study analyzed the clinical characteristics and outcomes of COVID-19 in adult hematopoietic stem-cell transplantation (HSCT) recipients through a systematic review and meta-analysis. The findings showed that symptomatic infection of SARS-CoV-2 was common in HSCT patients and the mortality rate was higher compared to the general population. It is crucial to implement measures to mitigate the risk of SARS-CoV-2 infection in this population and carefully evaluate HSCT recipients who develop COVID-19.

Strongyloides infection screening in transplant candidates: What is the best strategy?

Summary: This study aimed to observe S. stercoralis infection in transplant candidates using different techniques, and propose an algorithm for the detection of this infection. Parasitological techniques found that 10% of fecal samples were positive. Anti-Strongyloides antibodies IgG were detected in 19.3% and 20.7% of patients by immunofluorescence assay and enzyme-linked immunosorbent assay, respectively. S. stercoralis DNA was observed in 17.3% of samples by conventional polymerase chain reaction and 32.7% of samples by quantitative polymerase chain reaction (qPCR).

Diabetes related phenotypes and their influence on outcomes of patients with corona virus disease 2019 (COVID-19)

Summary: This study analyzed the differences between diabetic and non-diabetic COVID-19 inpatients. The results showed that diabetic patients were more likely to have abnormal glucose metabolism, inflammatory response, and organ damage, and had a higher mortality rate. Factors related to blood glucose levels, age, gender, and BMI also influenced the mortality risk in diabetic patients.

Summary: Streptococcus constellatus, a gram-positive coccus member of the Streptococcus anginosus group, can be found in the oral flora and commonly cause abscesses in the gastrointestinal tract, lungs, and heart. While Streptococcus species other than S. pneumoniae are rare causes of brain abscesses, this case report highlights a severe case of extra and intracranial abscesses caused by S. constellatus in an immunocompetent host.
