Rivaroxaban postmarketing risk of liver injury

Background & Aims: Rivaroxaban is an oral direct factor Xa inhibitor that has been marketed worldwide since 2008 for the primary and secondary prevention and treatment of thromboembolic disorders. Although liver injury was observed in premarketing trials of rivaroxaban, there are no published postmarketing cases of liver injury associated with rivaroxaban. Methods: Report of 14 cases of liver injury associated with rivaroxaban, including two with liver biopsy, and search queries in three large international pharmacovigilance databases for comparable cases. Results: Formal causality assessment classified rivaroxaban as the highly probable, probable, and possible cause in 4, 7, and 3 patients, respectively. Search results from three large international pharmacovigilance databases revealed a considerable number of additional hepatic adverse events where rivaroxaban was reported as a suspected cause. Conclusions: We interpret the presented information as a relevant safety signal that should be followed by pharmacoepidemiological studies in order to reliably estimate absolute and relative risks of liver injury associated with rivaroxaban in support of rational risk-benefit assessment. Meanwhile, incident symptoms and signs of liver disease in patients treated with rivaroxaban should be considered as a potential adverse drug reaction, and if no other likely cause can be identified rivaroxaban should be stopped as soon as possible. Characterization of Novel Fluorescent Bile Salt Derivatives for Studying Human Bile Salt and Organic Anion Transporterss

Summary: Synthetic modification of common bile acids by attaching small organic fluorescent dyes to the bile acid side chain resulted in novel fluorescent probes that interact with hepatic and intestinal organic anion transporters (OATP1B1, OATP1B3, OATP2B1), bile salt uptake transporters (NTCP, ASBT), and bile salt efflux transporters (BSEP, MRP2). Although the fluorescent bile acid derivatives are taken up into cells via the OATPs, the efflux transporters do not transport any of them except one.

Structures of ABCB4 provide insight into phosphatidylcholine translocation

Summary: ABCB4 is expressed in hepatocytes and plays a key role in translocating phosphatidylcholine into bile canaliculi. Cryogenic electron microscopy structures of ABCB4 reveal its specificity for phosphatidylcholine and the essential role of key residues in this process. These findings provide a structural basis for understanding bile generation and its sensitivity to azole drugs.

Cloning and characterization of a novel functional organic anion transporting polypeptide 3A1 isoform highly expressed in the human brain and testis

Summary: In this study, the cloning and functional characterization of a third isoform of OATP3A1, OATP3A1_V3, was described. OATP3A1_V3 showed widespread expression in human organs, with the highest expression in the brain and testis. Immunostaining and functional assays confirmed the expression of OATP3A1_V3 in the human choroid plexus, neurons, and both germ and Sertoli cells of the testis, and identified dehydroepiandrosterone sulfate as a novel OATP3A1 substrate.

Transporters for Bile Formation in Physiology and Pathophysiology

Summary: The liver plays vital roles in the body, including bile formation and detoxification. Bile salts, the main components of bile, are organic anions and can be cytotoxic at high concentrations. The liver's detoxification function exposes it to many harmful compounds, which can potentially damage the liver. Impaired bile formation can lead to the accumulation of bile salts in the liver and subsequently result in liver disease. Diagnosing liver diseases involves measuring liver function parameters. This overview focuses on the role of organic anion transporters in bile formation at the protein level under normal physiological conditions and in liver function tests used for diagnosing liver diseases in pathophysiological situations.

In this study, a new strategy was used to introduce bright fluorescent dyes into antibodies without affecting their functionality.
