Summary: Intrauterine growth restriction (IUGR) is associated with perinatal complications and long-term cardiovascular and renal diseases. This study investigates the effects of IUGR on renal and cerebral perfusion and oxygen saturation in term neonates. The results show that IUGR infants have left ventricular dysfunction, higher cerebral and renal oxygen saturation, and subclinical renal damage.

Acute and chronic kidney complications in children with type 1 diabetes mellitus

Summary: This cross-sectional study evaluated the performance of height-, age-, and BSA-based kidney length percentiles in identifying small kidneys and predicting reduced eGFR and/or elevated BP in children with and without overweight/obesity. The results showed that BSA-based percentiles underestimate kidney length in overweight/obese children, and it is recommended to use height-based or ideal body surface area-based percentiles.

Euthyroid sick syndrome and its association with complications of type 1 diabetes mellitus onset

Summary: This study aimed to evaluate the association between euthyroid sick syndrome (ESS) and severity indexes of type 1 diabetes mellitus (T1DM) onset, such as diabetic ketoacidosis (DKA) and kidney damage. It also examined the relationship between clinical/metabolic parameters at T1DM onset and thyroid hormones, and the prognostic role of ESS in delayed recovery from kidney damage. The results showed that ESS was associated with severity of T1DM onset and delayed recovery from acute kidney injury.

Association between non-alcoholic fatty liver disease and subclinical hypothyroidism in children with obesity

Summary: The study aimed to evaluate the relationship between non-alcoholic fatty liver disease (NAFLD) and thyroid function tests, testing if the relationship could be driven by obesity and insulin resistance (IR), and to explore the influence of PNPLA3 I148M and TM6SF2 E167K polymorphisms on the association between NAFLD and thyroid function in children with obesity. The results showed that children with NAFLD had a higher prevalence of subclinical hypothyroidism (SH), and patients with SH had a higher risk of NAFLD. Severe obesity and IR degree were associated with both NAFLD and SH. The TM6SF2 E167K polymorphism was found to have a protective role against SH in children with obesity and NAFLD.

The Impact of COVID-19 Pandemic Lockdown on the Relationship between Pediatric MAFLD and Renal Function

Summary: It is found in the study that both direct and indirect effects of COVID-19 are present in all age groups. The impact of the COVID-19 pandemic lockdown on the relationship between MAFLD and renal function in children with CKD due to CAKUT was investigated. The study showed that CKD patients with MAFLD had higher levels of BMI-SDS, serum uric acid, triglycerides, and microalbuminuria, and lower eGFR levels. Patients with MAFLD also had higher ferritin and white blood cell concentrations compared to peers without MAFLD.

Early-in-Life Serum Aldosterone Levels Could Predict Surgery in Patients with Obstructive Congenital Anomalies of the Kidney and Urinary Tract: A Pilot Study

Summary: The aim of this study was to determine whether early measurements of serum aldosterone levels or plasmatic renin activity (PRA) could predict the need for surgical intervention in infants with obstructive congenital anomalies of kidney and urinary tract (CAKUT). A total of 20 infants aged 1-3 months with suspected obstructive CAKUT were prospectively enrolled and followed up for 2 years. The results showed that infants who required surgery had significantly higher levels of aldosterone at 1-3 months compared to those who did not require surgery (p = 0.006). The ROC curve analysis identified a cut-off of 100 ng/dL for aldosterone, which predicted the need for surgery with 100% sensitivity and 64.3% specificity. However, PRA measured at 1-3 months was not a predictor of surgery. In conclusion, early measurements of serum aldosterone levels can serve as a predictor for surgical intervention in infants with obstructive CAKUT.

From Clinical Scenarios to the Management of Lower Urinary Tract Symptoms in Children: A Focus for the General Pediatrician

Summary: Lower urinary tract symptoms (LUTS) are highly prevalent in the pediatric population, with diurnal incontinence and nocturnal enuresis being the most common symptoms. The presence of constipation should be considered and treated as it is often curative for both incontinence and enuresis. Diurnal symptoms in nocturnal enuresis and urgency in diurnal incontinence aid in the differential diagnosis. Urotherapy is the first-line treatment, with oxybutynin and desmopressin used if the initial treatment is unsuccessful. It is important to identify conditions that can be potentially harmful to the kidneys and urinary tract, as LUTS may be the initial manifestation.

Summary: This study aimed to investigate the effect of LSS rs2254524 polymorphism on blood pressure in obese children and adolescents. The study found that individuals carrying the A allele had a higher risk of developing stage I and stage II hypertension. This risk was independent of confounding factors. Therefore, LSS rs2254524 worsens cardiovascular health in obese children and adolescents by increasing blood pressure.

Summary: The study aimed to evaluate the diagnostic performance of clinical and radiological signs (on voiding cystourethrography [VCUG]) to detect posterior urethral valves (PUV) in the post-neonatal period. 118 male patients were prospectively enrolled and direct and indirect PUV signs on VCUG were assessed. Only half of patients with endoscopy-confirmed PUV presents with direct sign of PUV on VCUG.
