Vitamin D deficiency and a CYP27B1-1260 promoter polymorphism are associated with chronic hepatitis C and poor response to interferon-alfa based therapy

Background & Aims: Vitamin D is an important immune modulator and preliminary data indicated an association between vitamin D deficiency and sustained virologic response (SVR) rates in hepatitis C virus (HCV) genotype 1 patients. We, therefore, performed a comprehensive analysis on the impact of vitamin D serum levels and of genetic polymorphisms with functional relevance within the vitamin D cascade on chronic hepatitis C and its treatment. Methods: Vitamin D serum levels, genetic polymorphisms within the vitamin D receptor and la-hydroxylase were determined in a cohort of 468 HCV genotype 1, 2, and 3 infected patients who were treated with interferon-alfa based regimens. Results: Chronic hepatitis C was associated with a high incidence of severe vitamin D deficiency compared to controls (25(OH)D-3 <10 ng/ml in 25% versus 12%, p <0.00001). 25(OH)D-3 deficiency correlated with SVR in HCV genotype 2 and 3 patients (50% and 81% SVR for patients with and without severe vitamin D deficiency, respectively, p <0.0001). In addition, the CYP27B1-1260 promoter polymorphism rs10877012 had substantial impact on 1,25-dihydroxyvitamin D serum levels (72, 61, and 60 pmol/ml for rs10877012 AA. AC, and CC, respectively, p = 0.04) and on SVR rates in HCV genotype 1, 2, and 3 infected patients (77% and 65% versus 42% for rs10877012 AA, AC, and CC, respectively, p = 0.02). Conclusions: Chronic hepatitis C virus infection is associated with vitamin D deficiency. Reduced 25-hydroxyvitamin D levels and CYP27B1-1260 promoter polymorphism leading to reduced 1,25-dihydroxyvitamin D levels are associated with failure to achieve SVR in HCV genotype 1, 2, and 3 infected patients. The role of serum sphingolipids as potential biomarkers of non-response to direct acting antiviral therapy in chronic hepatitis C virus infection

Summary: The serum concentration of sphingolipids, especially sphingosine, sphinganine, and their derivatives S1P and SA1P, along with glucosylceramides, can predict the failure of DAA treatment in chronic HCV patients. These sphingolipids could serve as additional biomarkers to guide treatment strategies aiming to eliminate HCV infection.

Angled-tip vs. straight-tip guidewire in ERCP: a randomized, multicenter study

The Conventional Weil Osteotomy Does Not Require Screw Fixation

Summary: The Weil osteotomy is commonly performed for chronic metatarsalgia, and screw fixation is historically used for metatarsal head refixation. However, this study suggests that screw fixation is not always necessary. Retrospective analysis of 155 patients with 278 Weil osteotomies demonstrated a lower revision rate in patients without screw fixation. Therefore, regular screw fixation after a Weil osteotomy may not be justified based on the available data.

A third dose of an mRNA vaccine is necessary to maintain neutralizing activity against SARS-CoV-2 and variants of concern-adapted versions of the vaccines are desired.

The APSANTICO Study: A Prospective Observational Study to Evaluate Antiphospholipid Antibody Profiles in Patients with Thromboembolic Antiphospholipid Syndrome (APS) after COVID-19 Infection and/or Vaccination

Summary: This prospective non-interventional trial included 82 patients with thromboembolic antiphospholipid syndrome (APS) and assessed the effect of COVID-19 infections and/or COVID-19 vaccinations on antiphospholipid antibodies (aPL). The study found no increase in aPL in the total study population, with slight decreases observed for certain antibodies and slight increases observed only in patients with COVID-19 infection and vaccination. The low recurrence rate of thrombotic events in these patients may be attributed to high vaccination rates and effective anticoagulation.

Sex difference in early mortality after abdominal aortic aneurysm repair

Summary: Although female patients have a lower prevalence of abdominal aortic aneurysm (AAA), they seem to have a worse treatment outcome compared with male patients. Both maximum aneurysm diameter and aortic size index (ASI) are important indicators of the risk of AAA rupture, among which ASI has been shown capable of equalizing sex-related anatomical differences.

A 23-Year Observational Follow-Up Clinical Evaluation of Direct Posterior Composite Restorations

Summary: The purpose of this study was to observe the quality of posterior composite restorations after more than 23 years of service. A total of 22 patients with 42 restorations underwent two follow-up examinations. Results showed significantly worse scores for six out of seven criteria at the second follow-up evaluation. The study suggests the need for further research with extended follow-up time and regular assessments.

Summary: Background & Aims: This study aimed to investigate the transmission and characteristics of recently acquired HCV infections (RAHCs) over a 10-year period. The majority of RAHC cases were found in HIV-coinfected men who have sex with men (MSM), and internationally connected transmission networks were observed in most patients. Spontaneous clearance rates were low, and reinfection rates increased in a subset of MSM patients with high-risk behavior.
