Organ donation and transplantation: a multi-stakeholder call to action

Raymond Vanholder, Beatriz Dominguez-Gil, Mirela Busic, Helena Cortez-Pinto, Jonathan C. Craig, Kitty J. Jager, Beatriz Mahillo, Vianda S. Stel, Maria O. Valentin, Carmine Zoccali, Gabriel C. Oniscu

Summary: While Europe has higher overall donation and transplantation activity compared to other continents, there are significant differences between European countries in various aspects of transplantation. Policy and clinical measures are necessary to increase access to transplantation and improve post-transplantation outcomes. Recommendations for advancements in organ donation and transplantation within the European Union are proposed in a Roadmap developed by key stakeholders in the field.

Outcomes of excessive alcohol drinkers without baseline evidence of chronic liver disease after 15 years follow-up: Heavy burden of cancer and liver disease mortality

Sonia Bernardo, Ricardo Crespo, Sofia Saraiva, Rui Barata, Sara Goncalves, Paulo Nogueira, Helena Cortez-Pinto, Mariana Verdelho Machado

Summary: The study shows that 6% of long-term heavy drinkers without evidence of chronic liver disease progressed to CLD, with cancer being the main cause of death. Abstinence for at least 1 year was positively associated with both CLD-free survival and overall survival.

Predictors for incomplete response to ursodeoxycholic acid in primary biliary cholangitis. Data from a national registry of liver disease

Helena Cortez-Pinto, Rodrigo Liberal, Susana Lopes, Mariana Machado, Joana Carvalho, Teresa Dias, Arsenio Santos, Claudia Agostinho, Pedro Figueiredo, Rafaela Loureiro, Alexandra Martins, Goncalo Alexandrino, Isabel Cotrim, Carina Leal, Jose Presa, Monica Mesquita, Joana Nunes, Catarina Gouveia, Ana Horta E. Vale, Ana Luisa Alves, Mariana Coelho, Luis Maia, Isabel Pedroto, Antonio Banhudo, Joao Sebastiao Pinto, Marta Vargas Gomes, Joana Oliveira, Valeska Andreozzi, Filipe Calinas

Summary: This study found a considerable proportion of patients with primary biliary cholangitis showed incomplete biochemical response to ursodeoxycholic acid (UDCA) treatment. Patients with cirrhosis, elevated levels of gamma-glutamyl transferase (GGT) and alkaline phosphatase (ALP) at diagnosis were identified as being at higher risk for incomplete response. Early identification of patients at risk of incomplete response could improve treatment care and guide clinical decision-making.

Summary: Despite comparable alcohol intake, AH patients presented with worse liver function compared with ndALD patients. Bilirubinostasis, severe fibrosis and ductular reaction were prominent features of AH. AH patients exhibited a more profound deregulation of gene expression compared with ndALD patients.

Summary: Non-alcoholic fatty liver disease (NAFLD) is a potentially serious liver disease that affects a significant portion of the global adult population, but has received little attention from the global public health community. This has led to a lack of inclusion of NAFLD in national and international strategies and policies for non-communicable diseases, highlighting the need for a more comprehensive public health response to the disease.

Do MAFLD Patients with Harmful Alcohol Consumption Have a Different Dietary Intake?

Sara Policarpo, Sofia Carvalhana, Ana Craciun, Ricardo Rios Crespo, Helena Cortez-Pinto

Summary: This study aimed to assess the dietary intake of metabolic-associated fatty liver disease (MAFLD) patients and explore the relationship between its inflammatory characteristics, liver fibrosis, and alcohol intake. The results showed that alcohol intake was associated with higher energy and certain nutrient intake, but there was no correlation between fibrosis severity or alcohol consumption and inflammatory characteristics.

A new framework for advancing in drug-induced liver injury research. The Prospective European DILI Registry

Summary: This study is the first multi-national prospective DILI registry study in Europe, and it revealed that antibacterials were the most commonly implicated drugs in drug-induced liver injury, while antineoplastic and immunomodulating agents accounted for a higher proportion than previously described. This European initiative provides an important opportunity to advance the study on DILI.

NAFLD, MAFLD and obesity: brothers in arms?

Mariana Verdelho Machado, Helena Cortez-Pinto

Summary: The year 2022 witnessed significant progress in both the NAFLD and obesity fields. Advancements in pharmacological treatment offer the potential for replacing bariatric surgery, while the application of artificial intelligence helps in evaluating histological effects of new drugs. Moreover, personalized screening for hepatocellular carcinoma in patients with NAFLD has also seen improvement.

Influence of COVID-19 on Patients with Esophageal Varices under Prophylactic Endoscopic Band Ligation Therapy

Ana Craciun, Ines Botto, Joao Lopes, Miguel Moura, Sofia Carvalhana, Helena Cortez-Pinto, Rui Tato Marinho

Summary: This study evaluated the impact of COVID-19 on the outcomes of patients receiving prophylactic endoscopic band ligation (EBL) therapy, including variceal eradication (VE), gastrointestinal bleeding (GI), and 6-month mortality. The results showed that COVID-19 had a negative effect on VE, and failure to achieve VE increased the risks of GI bleeding and 6-month mortality.

Predictive factors for long-term survival in patients with advanced hepatocellular carcinoma treated with sorafenib

Daniela Reis, Miguel Moura, Luis Carlos Freitas, Sofia Carvalhana, Paulo Jorge Nogueira, Raquel Gaio, Rui Tato Marinho, Helena Cortez-Pinto

Summary: A retrospective study of 77 advanced hepatocellular carcinoma patients found that Child-Pugh class A and absence of portal vein invasion were independent predictors of long-term survival. Sorafenib treatment can lead to over 2-year survival in a subgroup of patients.
