Clinical recommendations for posttransplant assessment of anti-HLA (Human Leukocyte Antigen) donor-specific antibodies: A Sensitization in Transplantation: Assessment of Risk consensus document

Summary: This consensus report provides clinical practice recommendations in kidney, heart, lung, and liver transplantation based on expert assessment of quality and strength of evidence regarding the relationship between anti-HLA donor-specific antibodies and transplant outcome. The recommendations address major clinical problems in transplantation, including the clinical implications of positive posttransplant DSA detection, the relevance of posttransplant DSA assessment for diagnosis and treatment management, and the relevance of posttransplant DSA for allograft prognosis and risk stratification.

Assessment and management of allosensitization following heart transplant in adults

Summary: Immunological injury, particularly caused by antibodies to de novo donor specific human leukocyte antigen (dnDSA) and antibody mediated injury and rejection, is a major challenge in heart transplantation. Current immunoassays are limited in unraveling the pathogenic potential of dnDSA, and the role of dnDSA is continuously evaluated. The timing and frequency of dnDSA monitoring, early detection, and risk mitigation, as well as management, remain challenging. A strategic approach that combines diagnostic assays, clinical presentation, and allograft injury/rejection is needed for tailored therapeutics. This review aims to outline the contemporary knowledge on detection, monitoring, and management of dnDSA after heart transplantation and propose a diagnostic and therapeutic approach to minimize morbidity and mortality while balancing adverse reactions from pharmacotherapy.

Detection and management of HLA sensitization in candidates for adult heart transplantation

Summary: Heart transplantation is the preferred therapy for advanced heart failure patients, but finding a suitable donor for sensitized candidates with human leukocyte antigen antibodies can be challenging. The increasing number of sensitized patients awaiting transplantation is likely due to the use of durable and mechanical circulatory support and the presence of congenital heart disease. This review discusses the assessment of human leukocyte antigen antibodies, desensitization strategies, donor selection methods, and induction management.

Donor hyperoxia is a novel risk factor for severe cardiac primary graft dysfunction

Summary: This study evaluated the risk factors for primary graft dysfunction (PGD) after heart transplant surgery. The excessive oxygen supplementation during donor management was found to be associated with an increased risk of severe PGD and early recipient death. Therefore, minimizing excessive oxygen supplementation is recommended in donor management.

Summary: This study assessed the efficacy and safety of hemodynamic-guided heart failure (HF) management in patients with elevated natriuretic peptides (NPs) but no recent HF hospitalization (HFH). The results showed consistent effects of hemodynamic monitoring in both patient groups, supporting the consideration of hemodynamic monitoring in the expanded group of chronic HF patients with elevated NPs.

Deficiency of the CD155-CD96 immune checkpoint controls IL-9 production in giant cell arteritis

Summary: Loss of function of inhibitory immune checkpoints can lead to autoimmune disease, such as giant cell arteritis (GCA). In this study, a defective CD155-CD96 immune checkpoint was found in GCA patients, resulting in the retention of CD155 in the endoplasmic reticulum (ER) of macrophages. Antigen-presenting cells with low CD155 expression induced the expansion of tissue invasive CD4+CD96+ T cells that released interleukin-9 (IL-9). The dysregulated immune response caused vessel wall destruction, which could be suppressed by anti-IL-9 antibodies in a GCA mouse model.

MST1 mediates doxorubicin-induced cardiomyopathy by SIRT3 downregulation

Summary: Heart failure is a common side effect of doxorubicin (DOX) treatment in cancer patients. This study aimed to investigate the role of the serine/threonine kinase MST1 in the development of DOX-induced heart failure. The results showed that MST1 signaling was activated in response to DOX treatment in mice and human myocardial tissue. Inhibition of MST1 attenuated DOX-induced cardiac dysfunction through the upregulation of SIRT3, a mitochondrial protection enzyme. These findings suggest that MST1 contributes to DOX-induced cardiomyopathy by downregulating SIRT3.

Impact of Carotid Stent Design on Embolic Filter Debris Load During Carotid Artery Stenting

Summary: This study compared the quantity of embolized material captured by filters during carotid artery stenting using different stent designs. The use of micromesh stents was associated with a lower embolization rate and embolic filter debris load, especially in patients with hypoechogenic and long plaques and preoperative evidence of asymptomatic ischemic cerebral lesion.

Summary: This study investigated the role of ClC-5, megalin, and cubilin in parietal epithelial cells and found that the number of hypertrophic parietal epithelial cells was significantly higher in lupus nephritis patients. The presence of these cells in Bowman's capsule may provide a potential resource for responding to protein overload in other glomerulonephritis.
