Summary: Hard-to-transfect cells, such as BMSCs, present challenges in intracellular delivery due to vesicle trapping. We found that coating nanoparticles with PDS1 can effectively avoid vesicle trapping in BMSCs and significantly enhance transfection efficiency and osteoblastic differentiation. Mechanistic studies suggest that higher cholesterol content in BMSCs' plasma membranes may contribute to the difficulty of vesicle escape.

Summary: Multimorbidity is common among people with T2DM, with the comorbidity profiles varying substantially. Two out of the top five comorbidities at diagnosis are traditional T2DM conditions, while the other three are not. The prevalence of each comorbidity increases over time, and patients with five or more comorbidities at diagnosis have a higher prevalence of each of the 35 comorbidities. People with obesity at diagnosis have significantly different comorbidity profiles, with the five commonest comorbidities being 50% more common in this group.

Summary: Patients with multimorbidity have a significant impact on healthcare systems, with higher mortality rates, polypharmacy, and development of various syndromes. Internists and Hospitalists are increasingly required to attend to these patients, but managing multimorbidity is complex and requires specific skills and experience. This paper proposes a set of emerging and re-emerging competences for internal medicine specialists to optimally address multimorbidity now and in the future.

Summary: The study aimed to analyze multimorbidity patterns among older individuals living in nursing homes and investigate their association with mortality, taking into account functional status. The results showed that different multimorbidity patterns were differently associated with mortality, with the heart diseases and dementia with sensory impairments patterns being associated with increased mortality risk. As the specific multimorbidity expression increased, the risk of mortality also increased.

Summary: Patients suffering from fibromyalgia often feel their testimonies are not taken seriously by healthcare professionals, resulting in epistemic injustice. This injustice is attributed to a gap between patients' lived experience of illness and professionals' conceptualization of illness, potentially harming patients' capacity as knowers. The dominant model of evidence-based practice in medicine may further devalue patients' stories, highlighting the need for more inclusive and patient-centered care approaches.
