Summary: This study of heart/kidney transplant using HCV+ donors found that while there may be a potential increased risk of early postoperative renal dysfunction, the results suggest that HCV+ donors are an important source of transplantable organs due to similar survival rates and decreased waitlist time.

Summary: Corneal neurotization is a promising surgical technique for treating moderate to severe neurotrophic keratopathy. It aims to restore corneal sensation by transferring healthy nerves to the anesthetic cornea. This review provides an overview of the current state of development, evidence, and experience in the field, emphasizing the need for consensus in reporting outcomes and validating outcome parameters.

Summary: In the current era, Fontan patients have a high mortality rate and often require transplantation. Heart transplant can be successful, providing high survival rates for both children and adults. However, early Fontan failure is difficult to rescue with transplantation and there are challenges in referring and treating adult patients. Limited data on referral and mortality rates are needed to better understand the overall situation and improve the quality of transplantation. Recent revisions in listing prioritization aim to address the specific needs of the Fontan population. Prioritizing high-risk children and improving mechanical support options can help reduce post-listing mortality.

Summary: This study provides an extensive analysis of the current practice of composite tissue allotransplantation, specifically face transplantation, including pre-procedural, intra-operative, and post-procedural settings, indications, contraindications, outcomes, ethical considerations, and future perspectives. The study highlights the importance of combining the latest achievements in FT with cutting-edge regenerative medicine procedures and innovative immunological processing. Strong international networks between FT experts are crucial for achieving higher-level outcomes and reducing complication rates, while caution is required in patient selection, clinical assessment, strict follow-up, and rejection management.

Summary: Pancreatic neuroendocrine tumors (pNETs) are rare and heterogeneous. Surgery is the main treatment modality, even for tumors with vascular involvement. Recent studies show that aggressive surgery with vascular reconstruction can be safely performed with good long-term survival. Accurate preoperative assessment is crucial for surgical treatment.

Summary: This study reviewed the clinical data of patients undergoing surgical portosystemic shunt procedures and compared the characteristics and outcomes between patients with portomesenteric venous thrombosis (PMVT) and those with cirrhosis. The results showed that while the two groups of patients were similar in many aspects, the rate of graft thrombosis was higher in patients with PMVT, and patients with PMVT had significantly lower patency and survival rates compared to those with cirrhosis.

Summary: Parotidectomy in pediatric patients for malignant neoplasms carries a significantly higher risk of nerve injury compared with parotidectomy for benign neoplasms. Parotidectomy for vascular malformations is associated with a significantly higher risk of bleeding requiring transfusion, while parotidectomy for lymphatic malformations has the lowest risk of adverse events.

Summary: The shortage of pediatric multivisceral donors leads to long waiting times and high mortality rates for pediatric patients. The use of donors after cardiac death, particularly with normothermic regional perfusion, has increased for solid organ transplantation, except for the intestine. This case demonstrates the possibility of reducing waiting time for patients through multivisceral transplantation from donors after cardiac death.

Summary: Hip arthroscopy patients can be broadly categorized into younger patients with femoroacetabular impingement, microinstability- or instability-related patients, patients with predominant peripheral compartment disease, and older patients with femoroacetabular impingement plus peripheral compartment disease based on clinical features. Older patients can achieve outcomes similar to younger patients in the absence of degenerative articular cartilage changes. With proper patient selection, hip arthroscopy may lead to durable and significant improvements, despite a potential for greater conversion rate to hip arthroplasty in older age group.

Summary: The update in liver allocation from DSA to AC had varying impacts on waitlist mortality and transplant probability for patients listed for combined liver-intestine transplantation, with lower transplant probability observed in adults but similar outcomes in pediatric patients. Additionally, there was a decrease in the proportion of patients who underwent transplant with exception points post-AC. Efforts should be made to ensure equitable organ allocation in this vulnerable patient population.

Summary: This review focuses on the unique aspects of pediatric intestinal and multivisceral transplantation (ITx, MVTx), including the evaluation process and surgical challenges specific to children. Recent studies have shown improvements in long-term patient and graft survival rates, but long-term graft function still remains a major challenge. The review also discusses post transplantation outcomes and quality of life.

Summary: Cervical spine deformity is a challenging condition that can be effectively treated with a pedicle subtraction osteotomy (PSO). Classifying and quantifying the alignment mismatch is crucial in determining the need for a PSO. Understanding the relative contraindications and associated outcomes is also important. Summary: This study examines the indications, surgical outcomes, and functional outcomes of robotic or endoscopic facelift thyroid surgery (FTS), and whether FTS yields comparable results to other surgical approaches. The findings indicate that FTS is a safe and effective approach for both benign and malignant thyroid lesions, with similar complications to conventional thyroidectomy and high cosmetic satisfaction.

Summary: This study describes a transvitreal biopsy technique for uveal melanoma and evaluates its safety and efficacy for obtaining prognostic genetic profiling samples. The results show that this technique can be used for small or posterior tumors, as well as for small anterior tumors where other biopsy approaches may be risky. The complications during the biopsy were minor and transient, and adequate tissue samples for gene expression profiling were obtained in all cases.

Summary: This study evaluated the relationship between the extent of allograft necrosis in liver biopsy and patient survival after liver transplantation. It was found that a higher extent of necrosis was associated with a higher allograft failure rate, with more than 50% necrosis significantly increasing the risk of failure. The MELD scores were not always accurate in predicting fatality caused by massive necrosis.

Summary: This study compared the effects of different inhaled anesthetics on early ischemia-reperfusion injury and clinical outcomes in pancreas allograft recipients. The findings suggest that sevoflurane and desflurane may be associated with lower peak amylase and lipase levels postoperatively in pancreas transplantation, with equivalent short- and long-term clinical outcomes for the three agents.

Summary: Advancements in critical care management have improved pediatric liver transplant outcomes, but there is a lack of specific guidelines for immediate post-LT care. A survey of active US centers shows practice variations in staffing, team composition, and anticoagulation for pediatric LT patients. There is marked inconsistency across centers in regards to team composition and immediate post-LT management, highlighting the need for a national consensus on post-LT PICU practices.

Summary: The liver is a common primary tumor site, responsible for about 5% of all cancers and over 8% of cancer-related deaths worldwide. Hepatocellular carcinoma (HCC) is the most prevalent type of liver cancer, accounting for approximately 75% of all primary liver tumors. Liver transplantation is a major therapeutic tool in the treatment of HCC, but it faces challenges of tumor recurrence and graft rejection. Cell-free DNA has emerged as a new tool for decision-making in the context of HCC treatment, and this article summarizes its main aspects, exploring its strengths and weaknesses.

Summary: This study retrospectively analyzed records of liver transplantation in infants younger than 1 year old at Indiana University from 2016 to 2022. The results showed that SLT may have better transplant outcomes compared to WLT in small donors and recipients. Therefore, SLT should be considered as a strategy to reduce waitlist times for infants in the absence of small, deceased donors for WLT.
