Summary: The novel FESDC strategy appears to be a safe and feasible option for the permanent endoscopic closure of gastrointestinal fistulas, with a moderate success rate in the long term, even for patients with previous failed endoscopic therapy. Further studies are needed to determine the role of this technique in the management of chronic gastrointestinal fistulas.

Summary: A tracheoesophageal fistula (TEF) is a pathological connection between the trachea and esophagus, which can occur congenitally or be acquired. Symptoms of TEF include choking with food intake, productive cough, pneumonia, or failure to thrive. OTSC has emerged as an effective treatment option for TEF, providing closure and improving the patient's quality of life.

Summary: This study aimed to evaluate the efficacy of endoscopic OTSC fixation of SEMS in preventing migration. The results showed a lower risk of migration with the use of OTSC fixation compared to no fixation. Further prospective multicenter studies should be conducted to validate the role of OTSC in preventing SEMS migration.

Summary: This study compared the closure strengths and leak pressures of the PSS and OTSC techniques in an ex vivo porcine model. The results showed that OTSC demonstrated greater closure strength and leak pressure compared to PSS. However, further research is needed to develop a suitable therapeutic strategy for larger defects due to the size limitations of OTSC.

Summary: This study found abnormal expression of miRNAs in the cerebrospinal fluid of patients with PSP. Particularly, the expression levels of miR-204-3p, miR-873-3p, and miR-6840-5p were significantly upregulated or downregulated in early stage PSP patients. These miRNAs are associated with the ubiquitin-proteasome system and autophagy pathway, suggesting their involvement in the pathogenesis of PSP.

Summary: Limited information is available on the efficacy and safety of endoscopic resection (ER) of gastric submucosal tumors (SMTs) in Japanese endoscopic practice where ER for gastric tumors is extremely popular. The study aimed to elucidate the current scenario of ER for gastric SMTs in Japanese endoscopic practice. Results showed an increasing trend in ER performance for gastric SMTs in Japan, with high feasibility and common pathology of gastrointestinal stromal tumors, without recurrence observed during the follow-up period.

Summary: The benefit of preoperative treatment followed by pancreatic resection in older patients with PDAC remains unclear. This study showed that preoperative treatment followed by curative resection could be safely performed in older patients, with similar outcomes as younger patients. Summary: This study established a new duodenal examination protocol called the Seven Pictures Rule (7PR), and investigated its feasibility and ability to detect neoplasms. The results showed that the completion rate of 7PR was 81.1%, and the overall detection rates of neoplasms, adenomas, and carcinomas were 0.84%, 0.71%, and 0.06% respectively. Most neoplasms were detected contralateral to the ampulla (69.2%), and the fewest were found in the transverse duodenum (0%). The completion rates for standard and ultrathin endoscopes were 84.4% and 65.6% respectively.
