Quantification of hepatic steatosis: A comparison of the accuracy among multiple magnetic resonance techniques

Background and AimMagnetic resonance imaging (MRI) and magnetic resonance spectroscopy (MRS) are important diagnostic tools for the non-invasive assessment of hepatic steatosis (HS). This study was conducted to compare different magnetic resonance (MR) techniques and correlate the MR findings with histological and intracellular lipid density findings. MethodsIn this institutional review board-approved, Health Insurance Portability and Accountability Act-compliant prospective study, 60 patients scheduled for liver resection were included in this study. Fat fraction in the non-tumorous liver parenchyma was estimated using double-echo MRI, triple-echo MRI (TE-MRI), and MRS. HS was defined by the histologic steatosis percentage (HSP), and intrahepatocellular triglyceride density (IHTGD) of the surgical specimen used as the reference standard. Imaging quantification results were evaluated using Pearson's correlation. Lin's concordance coefficient and Bland-Altman 95% limits of agreement were used to evaluate the concordance of IHTGDs estimated by the three MR techniques. The diagnostic performance was compared using receiver operating characteristic curve analysis. ResultsHS assessed by TE-MRI and MRS had a stronger relationship with HS assessed by HSP and IHTGD. The TE-MRI method had the highest concordance correlation coefficients (=0.881) and percentage (95%, 57/60) within the Bland-Altman 95% limits of agreement. Receiver operating characteristic curve analysis for diagnosing >5% HSP showed significantly larger area under the curve (0.9783) for TE-MRI than for double-echo MRI (0.8773, P=0.0121). ConclusionsAmong the three MR techniques, TE-MRI and MRS may be the preferred techniques for non-invasive assessment of HS.

Interval dynamics of transplantability for hepatocellular carcinoma after primary curative resection: risk factors for nontransplantable recurrence

Summary: This study investigated the changes in transplantability between primary and recurrent Hepato-cellular carcinoma (HCC) after hepatic resection (HR) and identified risk factors for nontransplantable recurrence (NTR). The study found that tumor diameter, resection extent, pathological grade, microvascular invasion, and early recurrence were associated with NTR. The overall transplantable pool increased after HR, suggesting that liver transplantation may not be necessary for some patients.

Longitudinal analysis of transplant candidates with primary sclerosing cholangitis in an Asian liver transplant center

Summary: Primary sclerosing cholangitis (PSC) is a rare disease in Asia and little research has been conducted on this ethnic group, especially in wait-listed patients for liver transplantation (LT). This study aimed to investigate the prognosis and outcomes of wait-listed patients with PSC in an Asian transplant center. The results showed that female PSC patients had higher AST to platelet ratio index scores and bilirubin levels compared to males, and were more likely to present with decompensating events. Female gender and LT were identified as protective factors for overall survival in multivariable analysis.

Comparison between non-surgical and surgical management of rib fractures in major trauma patients without brain injuries

Summary: This study aimed to evaluate the outcomes of non-surgical and surgical treatments in major trauma patients without brain injuries. The results showed that the surgical group had significantly improved pain scores and shorter hospital stays compared to the non-surgical group. Therefore, surgical management of rib fractures can have positive effects on pain and hospital stay in major trauma patients.

Viral Status and Treatment Efficacy in Recurrent Hepatocellular Carcinoma After Primary Resection

Summary: This retrospective study analyzed 726 patients who experienced intrahepatic recurrence after primary hepatectomy for hepatocellular carcinoma (HCC). The results showed that both radiofrequency ablation (RFA) and transarterial chemoembolization (TACE) were effective in improving long-term survival, particularly in patients with hepatitis B virus (HBV). Antiviral treatment complemented the survival of patients with hepatitis C virus (HCV) after RFA, especially in late first recurrence.

Summary: This study developed a simple and robust genetic classification for HBV-HCC, which divided it into 4 groups: G1 with HBV-TERT integration (25.5%); G2 with HBV-MLL4 integration (10.5%); G3 with TERT promoter mutation (30.1%); and G4 without these three mutations (34.0%). G3 had the highest male-to-female ratio and cirrhosis rate, and was associated with higher early recurrence and mortality after resection, while G4 had the best outcome.

Selection of consistent breath biomarkers of abnormal liver function using feature selection: a pilot study

Summary: Breath profiling is a non-invasive technique to identify biomarkers for various diseases. This study aimed to identify potential stable biomarkers in exhaled breath during different physiological states, including rest and brief workouts, to develop a non-invasive diagnostic tool for detecting abnormal liver function. Gas chromatography and mass-spectrometer quantified dataset was used for analysis, and machine learning techniques were employed for feature selection and model training.
