Predictive value of tumor markers for hepatocarcinogenesis in patients with hepatitis C virus

Increases in tumor markers are sometimes seen in patients with chronic liver disease without hepatocellular carcinoma (HCC). The aim of this study was to determine the relationship between the levels of three tumor markers [alpha-fetoprotein (AFP), Lens culinaris agglutinin-reactive fraction of AFP (AFP-L3%), and des-gamma-carboxy prothrombin (DCP)] and hepatic carcinogenesis to identify hepatitis C virus (HCV) carriers at high risk for cancer development. A total of 623 consecutive HCV carriers with follow-up periods of > 3 years were included. The average integration values were calculated from biochemical tests, and tumor markers, including AFP, AFP-L3%, and DCP, and factors associated with the cumulative incidence of HCC were analyzed. HCC developed in 120 (19.3%) of the 623 patients. Age > 65 years [adjusted relative risk, 2.303 (95% confidence interval, 1.551-3.418), P < 0.001], low platelet count [3.086 (1.997-4.768), P < 0.001], high aspartate aminotransferase value [3.001 (1.373-6.562), P < 0.001], high AFP level [a parts per thousand yen10, < 20 ng/mL: 2.814 (1.686-4.697), P < 0.001; a parts per thousand yen20 ng/mL: 3.405 (2.087-5.557), P < 0.001] compared to < 10 ng/mL, and high AFP-L3% level [a parts per thousand yen5, < 10%: 2.494 (1.291-4.816), P = 0.007; a parts per thousand yen10%: 3.555 (1.609-7.858), P < 0.001] compared to < 5% were significantly associated with an increased incidence of HCC on multivariate analysis. Increased AFP or AFP-L3% levels were significantly associated with an increased incidence of HCC. Among HCV carriers, patients with a parts per thousand yen10 ng/mL AFP or patients with a parts per thousand yen5% AFP-L3% are at very high risk for the development of HCC even if AFP is less than 20 ng/mL or AFP-L3% is less than 10%, which are the most commonly reported cutoff values.

Long-term outcomes of viral eradication in patients with hepatitis C virus infection and mild hepatic fibrosis

Summary: This study investigated the impact of antiviral therapy on clinical outcomes in patients with hepatitis C virus infection and mild liver fibrosis, showing that HCV eradication in patients with mild fibrosis reduces the risk of hepatocarcinogenesis and mortality. Furthermore, successful HCV eradication before the age of 50 may result in a prognosis similar to that of the age-matched and sex-matched general population.

Summary: Since its discovery in 1989, the road to a cure for hepatitis C virus (HCV) has been slow, but most patients can now expect to achieve a sustained virological response (SVR). Adverse events are rare or mild, and patients with compensated cirrhosis and other co-morbidities are often eligible for treatment. However, a small number of patients fail to eradicate the virus even after retreatment.

This study used data from 75.5 million blood donations from 2006 to 2020 to predict age-specific blood donation rates from 2021 to 2035 using an age-period-cohort model.

Incidence and prevalence of epilepsy in Japan: a retrospective analysis of insurance claims data of 9,864,278 insured persons

Summary: This study estimated the prevalence and incidence rates of epilepsy in Japan using nationwide insurance claims data. The results showed that over an 8-year period, a total of 77,312 individuals were diagnosed with epilepsy, resulting in a prevalence rate of 6.0 per 1,000. The incidence rate was 72.1 per 100,000 person-years, with higher rates among females. The highest incidence rates were observed among infants under 12 months and the elderly aged 70-74 years. These findings are important for informing health policies.

Burden of chronic hepatitis B and C infections in 2015 and future trends in Japan: A simulation study

Summary: This study determined the prevalence and genotype distribution of HBV infection in Cambodia through a systematic review with meta-analysis. The results showed a decrease in HBV infection prevalence in Cambodia, indicating the effectiveness of national hepatitis B immunization, improved clinical hygiene, and the use of disposable devices. However, the estimated HBV prevalence among the general population still remains at an intermediate endemicity level, highlighting the need for population screening, high vaccination coverage, health promotion, and HBV surveillance to meet the WHO 2030 goal.
