Identification of a human splenic marginal zone B cell precursor with NOTCH2-dependent differentiation properties

Mouse splenic marginal zone precursors (MZPs) differentiate into marginal zone B (MZB) cells under a signaling pathway involving Notch2 and its ligand, delta-like 1 ligand (Dll1). We report the identification of an MZP subset in the spleen of young children. These MZPs differentiate into MZ-like B cells in vitro in the presence of OP9 cells expressing human DLL1, as demonstrated by the up-regulation of classical MZB cell markers. A set of diagnostic genes discriminating IgM(+)IgD(+)CD27(+) blood and splenic MZB cells from switched B cells was identified (up-regulation of SOX7, down-regulation of TOX, COCH, and HOPX), and their expression during the induction assay mirrored the one of MZB cells. Moreover, Alagille patients with a NOTCH2 haploinsufficiency display a marked reduction of IgM(+)IgD(+)CD27(+) B cells in blood, whereas their switched memory B cells are not affected. Altogether, these results argue in favor of the existence of a rodent-like MZB cell lineage in humans.

Congenital diaphragmatic hernia in Coffin Siris syndrome: Further evidence from two cases

Summary: Coffin-Siris Syndrome (CSS) is a rare genetic condition characterized by nail and finger bone abnormalities, coarse facial features, excessive hair growth, developmental delay, intellectual disability, and growth impairment. This study presents two cases of CSS with congenital diaphragmatic hernia (CDH) and identifies different causative genetic variants. The occurrence of CDH did not predict CSS in these cases, but prompted genetic testing. This study provides further evidence of the association between CSS and CDH.

DSP-Related Cardiomyopathy as a Distinct Clinical Entity? Emerging Evidence from an Italian Cohort

Summary: Variants in desmoplakin gene (DSP) are usually associated with arrhythmogenic cardiomyopathy (ACM) or dilated cardiomyopathy (DCM) inherited in an autosomal dominant manner. This study analyzed a cohort of 18 probands with heterozygous DSP variants and identified 16 pathogenic or likely pathogenic variants. Patients generally presented as asymptomatic or mildly symptomatic, and cardiac magnetic resonance imaging revealed left ventricular myocardial fibrosis. These findings confirm that DSP-related cardiomyopathy is a distinct clinical entity characterized by a high arrhythmic burden, variable degrees of left ventricular enlargement, and myocarditis-like episodes.

Novel Insulin-Like Growth Factor 1 Gene Mutation: Broadening of the Phenotype and Implications for Insulin Resistance

Summary: This study describes a 12.6-year-old girl with severe short stature and insulin resistance, but with normal hearing and neurological development. Whole exome sequencing revealed a novel IGF1 mutation, leading to low immunoreactive IGF1 levels in serum and significantly reduced bioactivity. The absence of significant hearing and neurodevelopmental involvement in this case is unusual and broadens the clinical spectrum of IGF1 mutations.

T-independent responses to polysaccharides in humans mobilize marginal zone B cells prediversified against gut bacterial antigens

Summary: By sequencing the antibodies produced by B cells in humans vaccinated with Pneumovax, it was found that marginal zone (MZ) B cells play a significant role in T-independent (TI) immune responses. The study also showed that immunoglobulin M (IgM) components were already diversifying and amplifying before vaccination. These clones could be traced back mainly to MZ B cells, along with clonally related IgA+ and IgG+CD27+ B cells. Recombinant monoclonal antibodies derived from these large B cell clones were able to recognize various bacterial species in the gut, indicating the involvement of MZ and switched B cells in TI responses and their cross-reactivity with pneumococcus through somatic hypermutation.

Summary: This study retrospectively analyzed an international cohort of 103 cases with biallelic TTNtv. The results showed a correlation between TTNtv and severe myopathies and/or cardiomyopathies with a congenital or childhood onset, presenting clinical features such as fetal akinesia, arthrogryposis, facial dysmorphisms, joint, bone, and heart anomalies resembling complex syndromic phenotypes. The findings suggest that TTN should be carefully evaluated in prenatal diagnostic processes, aiming to improve diagnostic performance, expand knowledge, and optimize prenatal genetic counseling.

Summary: This study provides a comprehensive description of the phenotypic and genotypic spectrum of ARF1-related neurodevelopmental disorder. De novo variants in ARF1 were identified among 17 unrelated individuals, and the detailed phenotypes included intellectual disability, microcephaly, seizures and PVNH. The study confirms the role of ARF1 in an autosomal dominant syndrome with a phenotypic spectrum including severe ID, microcephaly, seizures and PVNH due to impaired neuronal migration.

Summary: This study reveals that AKT is involved in the formation of marginal zone (MZ) B cells in both mice and humans. By manipulating AKT signaling in B cells and its impact on FoxO transcription factors, the AKT-FoxO axis is identified as a switch for MZ B cell formation in mice. This developmental pathway is conserved in humans, where AKT-dependent splenic IgD+CD27+ B cells are proposed as the equivalent of MZ B cells.

Summary: We studied WDR5, a broadly studied key protein involved in various biological functions. We found rare de novo germline missense variants in WDR5 that are associated with neurodevelopmental disorders. These variants may disrupt interactions of WDR5 with crucial proteins such as RbBP5 and/or KMT2A/C.
