Summary: Background:Neuroendocrine tumors can cause ectopic Cushing syndrome, and most patients have metastatic disease at diagnosis. This retrospective study aimed to identify risk factors, evaluate management strategies, and explore the role of bilateral adrenalectomy in patients with ectopic Cushing syndrome secondary to neuroendocrine tumors. Results showed that patients with neuroendocrine tumors before ectopic Cushing syndrome development were more likely to have metastatic disease and worse survival. Medical control of hypercortisolism was challenging, and well-selected patients may benefit from bilateral adrenalectomy. Multidisciplinary management is essential in this complex disease.

Summary: This study compared the serum IGF-I dynamics and variability in patients treated with SRLs, surgically cured patients, and healthy controls. The results showed that IGF-I decreased significantly at Day 7 and Day 14 after injection in SRL-treated patients, but remained stable in cured patients and healthy controls. In addition, SRL-treated patients had higher IGF-I variability, especially in nonoptimally controlled patients. The measurement at the farthest distance from SRL injection was the best predictor of optimal disease control.

Summary: This study found that in type 2 diabetes patients, diastolic dysfunction is associated with impairment of endothelium-independent dilatation, but not with endothelium-dependent dilatation; cardiac autonomic dysfunction may be present in patients with more severe diastolic dysfunction. Therefore, further investigation is needed to explore the role of cardiac autonomic dysfunction in diastolic dysfunction in larger populations.

Summary: This study investigates the tumor immune infiltration in medically naive acromegaly patients and finds that CD8+ lymphocytes are positively correlated with CD4+ lymphocytes and CD68+ macrophages, while the CD8+/CD4+ ratio is negatively correlated with the CD68+/CD8+ ratio. CD8+ and CD138+ lymphocytes are more numerous in cases responsive to fg-SSA compared to nonresponsive cases. CD8+ lymphocytes act as a single predictor of response to fg-SSA treatment. The study suggests that lymphocytes and macrophages form an immune network in somatotropinomas and the characteristics of the immune infiltrate may predict treatment outcome.

Summary: This study reports three cases of resistant acromegaly treated with a de-escalation approach using pasireotide LAR. The results showed that de-escalation treatment with pasireotide LAR may allow a greater proportion of patients to achieve control of acromegaly, especially in clinically aggressive cases. Although there may be a risk of hyperglycemia, it can be controlled with oral antidiabetic medications.
