A P5 cancer medicine approach: why personalized medicine cannot ignore psychology

A system approach termed P4 medicine has recently been proposed in the field of oncology. This approach has been advanced as an extension of what is usually called 'personalized' or 'genomic medicine'. P4 medicine creates effective predictive, personalized, preventive and participatory models to treat patients. In order to give more relevance to the behavioural component that impinges on the way individuals act to prevent, cope and react to illnesses, how they decide between different therapeutic options and interact with physicians and adhere to treatment, we propose that P4 medicine should be transformed into P5 medicine. The fifth P represents the psycho-cognitive aspects to be considered in order to empower the patient, increase his/her quality of life and transform him/her from a passive recipient into an active decision-maker in the treatment process.

What people really change after genetic testing (GT) performed in private labs: results from an Italian study

Summary: The study investigated long-term changes in health-related choices, individual tendencies, and risk attitudes among Italian GT users. Results showed that participants who intended to modify their behavior after learning their GT results did so over time, with a healthier diet being the most common long-term behavioral change observed. Psychological variables like risk-taking attitude and risk tolerance did not seem to affect the decision to change lifestyle. Overall, there was a reduction in anxiety and worry over health, but also a decrease in motivation for health promotion and prevention, health esteem, and positive expectations for future health.

Summary: The study evaluated the work and psychological impact of COVID-19 on radiation therapy residents in Lombardy during the first 3 months of the outbreak. Results showed that most residents experienced increased workload and stress, with difficulties in sleep quality and concentration. Despite the availability of psychological support, none of the residents requested it. The majority of residents adapted to the new workplace scenario, highlighting some health risks and well-being challenges.

However, increasing global uptake requires further sensitization and awareness actions.

Summary: During an acute cardiac event, Takotsubo Syndrome (TTS) and Acute Coronary Syndrome (ACS) exhibit similar clinical characteristics. This exploratory research found that post-recovery TTS patients differ from ACS patients in their level of concern about their health and their need for reassurance and information. Additionally, only TTS patients reported the presence of a significant emotional trigger preceding the acute cardiac event.

Summary: This study explored risk estimations related to COVID-19 perception and distress in oncologic outpatients undergoing active hospital treatments compared to the general population. The results showed that cancer patients have a lower risk perception and a more positive illness representation related to COVID-19 compared to control subjects. The level of distress is not associated with the clinical status, but it is moderated by illness perception.

Summary: This study analyzed the prevalence of psychological symptoms and their determinants in different categories of healthcare workers during the COVID-19 pandemic. The results showed that anxiety and depressive symptoms did not reach the pathological threshold, but post-traumatic and burnout symptoms tended toward the critical threshold, especially in physicians. Perceived stress was found to be the main determinant of psychological distress.

Summary: Little is known about physiological and psychological responses to mental stress in stable patients affected by pulmonary arterial hypertension (PAH). The current explorative controlled pilot study found that there were no significant differences in heart rate and perceived stress between PAH patients and healthy subjects during standardized mental stress testing. However, there was a significant correlation between heart rate and perceived stress.

The self's choice: Priming attentional focus on bodily self promotes loss frequency bias

Summary: Previous research suggests that focusing on self-representations enhances the ability to evaluate internal sensations. This study analyzed the decision-making process under various conditions of stimulated self-representations and found that participants tended to choose losses more frequently when primed with bodily self-representations. This could be due to the distractive nature of bodily self-focus, diminishing participants' long-term outcome commitment, or the enhanced interoception leading to aversion to losses.

Cancer patients' participation and commitment to psychological interventions: a scoping review
