Local impact of thyroid hormone inactivation

Deiodination is a critical process by which the minimally active thyroxine (T(4)) molecule is converted into the favorite ligand for thyroid hormone (TH) receptors, triiodothyronine (T(3)). The iodothyronine deiodinases type 1, 2, and 3 (D1, D2, and D3) constitute a potent mechanism of TH activation (D1 and D2) or inactivation (D3), which functions by tissue specifically regulating TH bioavailability. D2 and D3 are widely expressed and in a dynamically and tightly coordinated fashion, thereby allowing cells to customize their own TH activity. D3, the major T(3) and T(4) inactivating deiodinase, catalyzes their conversion to 3,3'w-diiodothyr-onine and to reverse T(3) respectively. According to common wisdom, D3 plays a major role in lowering serum TH concentrations during development, as supported by the much wider D3 tissue expression in the embryo structures than in the adult tissues. However, several recent studies show that D3 is reexpressed in adult life in various pathophysiological contexts, which strengthens the concept that cell-specific TH inactivation is a critical mediator in cellular TH metabolism. This review focuses on the progress made in understanding the physiological function and significance of D3. It summarizes the intriguing evidence that D3 plays a pivotal role in defining local TH concentration in the developing fetus and in several conditions in adult life. Summary: Hormones play a key role in cancer development, and the alteration of intratumoral thyroid hormone concentration is a common feature in many human tumors. Deiodinases are crucial for cell-autonomous control of TH action, and their expression is often altered in tumors. Studies have shown a dynamic expression of deiodinases in cancer cells, influenced by a network of factors, which exposes cancer cells to varying concentrations of TH with functional implications on cellular functions.

The importance of the RET gene in thyroid cancer and therapeutic implications

A clinical case highlighted the successful treatment of a melanoma patient with the SMO p.Gln216Arg mutation using imiquimod, shedding light on a potentially druggable target for melanoma patients.

Thyroid Hormone Action in Muscle Atrophy

Summary: Inactivation of Dio3 in brown adipose tissue during embryonic development leads to premature exposure to thyroid hormone (T3) signaling, resulting in early adipocyte development and gene expression. BAT-D3KO mice show changes in weight, adipocyte area, and gene expression related to mitochondria, fat, and glucose homeostasis. However, their energy expenditure and ability to maintain core temperature remain normal.

Vandetanib downregulates type 2 deiodinase in fibro/adipogenic progenitors

Summary: In this study, the authors investigated the effect of vandetanib, a multi-kinase inhibitor, on the expression and activity of deiodinase enzymes responsible for thyroid hormone metabolism. They found that vandetanib specifically downregulated the expression of type 2 deiodinase and reduced its activity in fibro/adipogenic progenitor cells. These findings provide insights into the mechanisms by which TKIs induce hypothyroidism and identify a specific cell population involved in this effect.

Type 2 deiodinase is expressed in anaplastic thyroid carcinoma and its inhibition causes cell senescence

Summary: D2 is highly expressed in anaplastic thyroid cancer cells and is essential for their proliferation and invasiveness. Inhibition of D2 leads to cell growth arrest and senescence, as well as reduced migration and invasion. Mutated p53(72R(R248W)) can induce D2 expression in transfected papillary thyroid cancer cells. These findings suggest that D2 is a potential therapeutic target for the treatment of anaplastic thyroid cancer.

Divergent Thyroid Hormone Levels in Plasma and Left Ventricle of the Heart in Compensated and Decompensated Cardiac Hypertrophy Induced by Chronic Adrenergic Stimulation in Mice

Summary: Chronic hemodynamic overload of the heart can lead to compensatory or decompensated ventricular hypertrophy and heart failure, which is partly due to a reduction in cardiac thyroid-hormone (TH) action. Increased expression of the TH-inactivating enzyme deiodinase type 3 (D3) and reduced plasma TH levels contribute to the decrease in cardiac TH levels. The study showed that decompensated LV hypertrophy with diminished cardiac output is associated with a greater reduction of cardiac TH levels than compensated hypertrophy with maintained cardiac output, and changes in systemic TH homeostasis play a role in the impairment of ventricular function in decompensated hypertrophy.

Combination of Lenvatinib and Pembrolizumab as Salvage Treatment for Paucicellular Variant of Anaplastic Thyroid Cancer: A Case Report

Summary: The case study describes a rare variant of anaplastic thyroid cancer in a 54-year-old woman, who initially responded well to a combination treatment of surgery, radiation therapy, and chemotherapy, and later received lenvatinib and pembrolizumab with a sustained partial tumor response. Paucicellular ATC may exhibit less aggressive behavior initially but can progress rapidly over time, with the combination of lenvatinib and pembrolizumab showing effectiveness as salvage therapy for an extended period.
