Summary: This study aims to investigate the impact of immunosuppression on pulmonary function trajectory in patients with rheumatoid arthritis-associated interstitial lung disease (ILD). The results showed that treatment with immunosuppressive agents improved lung function after 12 months. The radiologic pattern of ILD and choice of immunosuppressive agent did not significantly affect pulmonary function trajectory.

Summary: This study compared the efficacy and immunogenicity of methotrexate (MTX) and azathioprine (AZA) as concurrent therapies for pediatric Crohn's disease (CD). The results showed that there were no significant differences in remission rates between the two medications, and they both effectively maintained adequate Infliximab (IFX) levels and reduced anti-drug antibody (ADA) production.

Summary: The study found that in patients with ulcerative colitis (UC) and Crohn's disease (CD), the gut microbiome was more similar to the oral microbiome, indicating increased ectopic gut colonization by oral bacteria in IBD patients. Incipient periodontitis did not significantly affect the clinical outcomes of UC and CD patients, but early periodontitis may be associated with worse clinical symptoms in some CD patients.

Summary: This case report highlights a Crohn's disease patient who developed toxoplasma gondii infection after treatment with azathioprine, with a genetic variation in NUDT15 but normal TPMP gene. NUDT15 variation may contribute to reduced tolerance to azathioprine and personalized therapy based on genetic testing may help reduce the risk of adverse events.

Summary: This study aimed to provide an update on the incidence of opportunistic infections (OIs) among adult IBD patients in randomized controlled trials (RCTs) of approved biologics and small-molecule drugs (SMDs). The study also aimed to describe the OI definitions utilized in RCTs and propose a standardized definition. The results showed that the different mechanisms of action may have specific OI profiles, and the lack of a uniform OI definition makes the estimates less reliable.

Summary: Inappropriate activation of Toll-like receptors and nucleotide-binding oligomerization domain receptors is involved in chronic disorders like inflammatory bowel disease (IBD). The stimulation of TLRs and NOD2 affects the activity and expression of Na+,K+-ATPase, which is related to electrolyte absorption imbalance and diarrhea in IBD patients.

Summary: Endoscopic submucosal dissection (ESD), endoscopic mucosal resection (EMR), and transanal minimally invasive surgery (TAMIS) are modern techniques that have significant roles in treating colorectal lesions. ESD allows for the resection of lesions of any size in a single piece, with advantages in oncological outcomes. EMR is a faster and accessible procedure but has limitations in en-bloc resection size. TAMIS is a minimally invasive surgical technique for rectal tumors, providing better visualization and minimal disruption. This article reviews the applications and evidence of these techniques in order to find the most suitable treatment for lesions in the rectum and anorectal junction.

Summary: Diagnosing and characterizing biliary strictures can be challenging. In this study, a deep learning-based algorithm was developed to identify and characterize malignant biliary strictures using digital single-operator cholangioscopy (D-SOC). The algorithm accurately detected malignant lesions and showed great potential for clinical use. This research highlights the potential of artificial intelligence in improving the diagnostic yield of D-SOC.

Summary: With the advance of the digital era, the use of AI for extracting valuable information from big data has become more common than we realize. Medical specialties, especially imaging techniques, have focused on incorporating AI tools for disease diagnosis and monitoring, although their practical application in clinics is still in its early stages. However, the introduction of these applications raises ethical issues concerning privacy, data protection, bias, explainability, and responsibility. This review highlights important bioethical concerns that need to be addressed before integrating AI solutions in healthcare protocols, with a specific focus on gastroenterology and capsule endoscopy.

Summary: Gastroenterology is increasingly using minimally invasive diagnostic methods, with capsule endoscopy becoming the first-line diagnostic approach for colon exploration. Adequate colonic preparation is crucial for accurate examinations, and our developed deep learning algorithm provides high performance in automatic classification. This algorithm is essential for the widespread adoption of capsule endoscopy and minimally invasive panendoscopy.
