Interspecies Transfer of blaIMP-4 in a Patient with Prolonged Colonization by IMP-4-Producing Enterobacteriaceae

A patient was colonized by IMP-4-producing Enterobacter cloacae and Escherichia coli strains for 7 months. IMP-4-producing E. cloacae strains were first and last isolated at day 33 and at 8 months after admission, respectively. IMP-4-producing E. coli strains were first and last isolated at days 88 and 181 after admission, respectively. The E. cloacae and E. coli isolates shared identical genetic features in terms of bla(IMP-4), bla(TEM-1), qnrB2, aacA4, HI2 plasmids, and ISCR1. This study shows the first prolonged colonization with in vivo interspecies transfer of bla(IMP-4).

Plasmid-Mediated Ciprofloxacin Resistance Imparts a Selective Advantage on Escherichia coli ST131

Summary: The plasmid-associated resistance gene, AAC (6')-Ib-cr, provides a selective advantage for the dissemination of Escherichia coli ST131, a highly antibiotic resistant clone. This gene, which inactivates the fluoroquinolone antibiotic ciprofloxacin, is present in over 70% of strains from the rapidly growing subgroup of multidrug resistant ST131. The presence of this gene gives ST131 an edge even in the presence of chromosomal FQ-resistance mutations.

However, there are variations in symptomatic treatment and antibiotic-sparing preventative strategies.

Summary: The incidence of Morganella morganii bloodstream infections is increasing in Queensland, Australia, with elderly male subjects and individuals with comorbidities at the highest risk. Although antibiotic resistance is not a major contributor to the current burden, ongoing surveillance is necessary.

Summary: This study compared the diagnostic value of the Shih-Yung conventional microscopic and urine flow cytometry methods in the detection of leukocyte and bacterial parameters of patients with urinary tract infections (UTIs). The results showed consistent agreement between the two methods, and urine flow cytometry demonstrated good performance in detecting acquired symptomatic UTIs.

Summary: This study developed instrument-free point-of-care methods using RPA technology and a simple lateral flow detection system to detect Neisseria gonorrhoeae and susceptibility to ciprofloxacin. The results showed that the developed assays were specific to N. gonorrhoeae and could effectively detect infections and antibiotic susceptibility.

Summary: The application of direct metagenomic sequencing from positive blood culture broth holds promise in solving the challenges of sequencing low-bacterial-load blood samples in sepsis patients. This study compared the identification of species and prediction of susceptibility using metagenomic sequencing and a machine-learning algorithm to conventional methods and other diagnostic platforms. The results showed that direct metagenomic sequencing from blood cultures is feasible for pathogen detection and susceptibility prediction, and further optimization is needed for uncommon species and complex resistance genotypes.

Epidemiology of Aeromonas Species Bloodstream Infection in Queensland, Australia: Association with Regional and Climate Zones

Summary: A retrospective population-based cohort study conducted in Queensland, Australia revealed that the incidence of Aeromonas species bloodstream infection (BSI) was relatively low in the region and did not show significant changes over a 20-year period. The study also found that the occurrence of Aeromonas species BSI varied significantly in different seasons, climate zones, and geographical locations, with higher rates observed in tropical north regions and among elderly males.

Duration of antibiotic treatment and timing of oral switching for bloodstream infections: a survey on the practices of infectious diseases and intensive care physicians

Summary: This study investigated the current practice of antibiotic treatment duration and timing of IV to oral switching for common bacteraemic conditions among Turkish infectious disease and ICU physicians. The results showed that the most commonly recommended duration of antibiotic treatment for bacteraemia was 14 days, followed by 10 and 7 days. Most physicians recommended switching to oral antibiotics after 7 days of IV treatment. Prolonged treatment durations may be due to a presumption that resistant bacterial infections require longer therapy.

Summary: An outbreak of vanB-type VREfm ST78 occurred in a large tertiary Australian hospital. Whole-genome sequencing revealed a complex transmission route involving unknown environmental reservoirs. This genomic analysis provided detailed information on the local epidemiology of the outbreak strain and facilitated better targeted control measures for VREfm.
