The results showed that infections caused by P. jirovecii mutants potentially resistant to sulfonamides are still rare events in Bulgaria, with DHPS genotype 1 being the predominant strain in the country.

Summary: The study aimed to identify potential risk factors and prognostic factors of Pneumocystis jirovecii pneumonia (PJP) infection in anti-melanoma differentiation-associated gene 5 antibody-positive DM (anti-MDA5+ DM) patients, and evaluate the diagnostic performance of metagenomic next-generation sequencing (mNGS). The results showed that the detection rate of PJP in anti-MDA5+ DM patients was 48.4%, and it often occurred in the first 3 months (68.2%) of the disease, with the highest mortality rate (20.5%). mNGS had a sensitivity of 100.0% and specificity of 90.0% in diagnosing PJP, with the highest area under the curve of 0.95.

Summary: This study aimed to establish the prevalence of Pneumocystis jirovecii infection in human hosts with different immune status and compare it with classical staining methods. The results showed that real-time PCR had higher sensitivity than staining methods, and the choice of a method for sample collection and examination played an important role in the efficiency of the laboratory diagnostics.

Summary: This study aims to describe a case series of patients with decompensated liver cirrhosis and Pneumocystis jirovecii pneumonia (PCP). The results show that liver cirrhosis can be an independent risk factor for PCP, especially in patients with severe alcoholic hepatitis who are on corticosteroid treatment for decompensated cirrhosis. Primary prophylaxis for PCP should be considered in these patients.

Summary: The risk factors for late-onset Pneumocystis jirovecii pneumonia (PCP) after liver transplantation (LT) have been analyzed in this study. Age >= 65 years at LT, cytomegalovirus infection, steroid pulse therapy, hepatocellular carcinoma recurrence, and lymphocytopenia were identified as independent risk factors for PCP. Targeted prophylaxis considering these risk factors could improve the prevention of this potentially lethal complication.

Summary: Respiratory cytomegalovirus (CMV) coinfection at diagnosis of Pneumocystis jirovecii pneumonia (PcP) is independently associated with increased 90-day mortality, specifically in HIV-negative patients.
