Nosocomial Outbreak of a Non-Cefepime-Susceptible Ceftazidime-Susceptible Pseudomonas aeruginosa Strain Overexpressing MexXY-OprM and Producing an Integron-Borne PSE-1 β-Lactamase

Cefepime (FEP) and ceftazidime (CAZ) are broad-spectrum cephalosporins that display similar MICs for wild-type Pseudomonas aeruginosa strains. Recently, P. aeruginosa isolates showing a discordance in susceptibility to CAZ and FEP have been noted at the Hospital de Bellvitge in Barcelona, Spain, and a clustering was suspected. During the study period (March to December 2007), 51 patients, particularly those in an intensive care units (ICUs) (n = 29 [57%]), infected or colonized with at least one P. aeruginosa non-FEP-susceptible and CAZ-susceptible (Fep(ns) Caz(s)) phenotype strain were detected. Twenty-three (45%) patients were infected, and the respiratory tract was the most frequent site of infection. Changes in the consumption of antimicrobials in the ICUs were observed over time: a progressive reduction in the levels of consumption of carbapenems (247 defined daily doses [DDD]/1,000 patient days to 66 DDD/1,000 patient days; P = 0.008), after restriction of its use in 2006, and an expected increase in the rate of piperacillin-tazobactam use (42 DDD/1,000 patient days in 2004 to 200 DDD/1,000 patient days in 2007; P < 0.001). Throughout the whole study period, only a single clone of a P. aeruginosa Fep(ns) Caz(s) phenotype strain was identified by pulsed-field gel electrophoresis analysis to be associated with the hyperexpression of MexXY-OprM and the production of an integron-borne PSE-1 beta-lactamase. In conclusion, we identified an epidemic P. aeruginosa clone of an Fep(ns) Caz(s) phenotype strain involving 51 patients, in particular, ICU patients. The combination of the overexpression of an efflux pump and PSE-1 beta-lactamase production is associated with the multidrug-resistant phenotype. The dominant use of a single class of antibiotics could have provided the selective pressure required for the emergence and spread of this P. aeruginosa strain.

Comparative analysis of in vitro dynamics and mechanisms of ceftolozane/tazobactam and imipenem/relebactam resistance development in Pseudomonas aeruginosa XDR high-risk clones

Summary: The study found that resistance development to imipenem/relebactam was slower and reached a lower level compared to ceftolozane/tazobactam in all tested XDR strains. Additionally, imipenem/relebactam resistance remained low even in ST175 isolates that had developed resistance to ceftolozane/tazobactam. Lineages evolved in the presence of ceftolozane/tazobactam exhibited high-level resistance, hypersusceptibility to imipenem/relebactam, and low fitness cost, while lineages evolved in the presence of imipenem/relebactam showed moderate resistance, no cross-resistance to ceftolozane/tazobactam, and high fitness cost.

Summary: This study aimed to compare the noninferiority of fosfomycin to ceftriaxone or meropenem in the treatment of bacteremic urinary tract infections caused by multidrug-resistant Escherichia coli. The results showed that there was no significant difference in clinical and microbiological cure rates between the fosfomycin group and the comparator group, but the rate of adverse event-related discontinuations was higher in the fosfomycin group.

Summary: A study conducted in Catalonia, Spain, analyzed the changes in catheter-related bloodstream infections (CRBSI) over a 12-year period. The study found a decrease in CRBSI associated with central venous catheters (CVC) and diagnosed in intensive care units (ICU), while the incidence of CRBSI related to peripheral venous catheters (PVC) and peripherally-inserted central venous catheters (PICVC) increased, especially in medical wards. The findings suggest the importance of implementing preventive measures in conventional wards to reduce CRBSI.

Penicillin-Binding Protein 5/6 Acting as a Decoy Target in Pseudomonas aeruginosa Identified by Whole-Cell Receptor Binding and Quantitative Systems Pharmacology

Summary: Beta-lactam antibiotics have been effective against susceptible Pseudomonas aeruginosa, but their penetration ability and binding to penicillin-binding proteins (PBPs) in intact bacteria have not been well studied. This research investigated the time course of PBP binding and target site penetration of 15 compounds in P. aeruginosa PAO1. The results showed that PBPs 1-4 were considerably bound in lysed bacteria, but the binding was attenuated in intact bacteria for slow penetrating beta-lactams. Imipenem showed the highest killing effect, while other drugs had lower killing rates. The study also found a correlation between PBP5/6 binding and the rate of net influx and PBP access.

Summary: This review discusses the evidence and therapeutic potentials of a soluble peptidoglycan-dependent signaling that can impact the virulence of Gram-negative bacteria. It highlights the need for new therapeutic options against antibiotic-resistant pathogens and explores the potential of targeting bacterial pathogenic power. Studies have suggested the existence of peptidoglycan-derived signaling capable of regulating bacterial behavior and these findings could be exploitable for therapeutic purposes. The review aims to gather and integrate the current knowledge gaps in order to enable the development of potential therapeutic strategies.

Impact of Peptidoglycan Recycling Blockade and Expression of Horizontally Acquired β-Lactamases on Pseudomonas aeruginosa Virulence

Summary: With the increasing antibiotic resistance, there is an urgent need for new weapons against top nosocomial pathogens like Pseudomonas aeruginosa. The interplay between beta-lactam resistance and virulence is promising for developing antivirulence therapies. In this study, the researchers demonstrated that peptidoglycan recycling blockade significantly attenuated the pathogenic power of P. aeruginosa strains hyperproducing AmpC beta-lactamase. However, the impact of peptidoglycan recycling disruption and different beta-lactamases on virulence varied. Cloning beta-lactamase OXA-2 derivatives were associated with outstanding virulence impairments, suggesting that mutations affecting the hydrolytic spectrum of these enzymes drastically impact the pathogenic power of P. aeruginosa.
