Neutrophil to Lymphocyte Ratio Predicts Poor Prognosis in Ischemic Cerebrovascular Disease

BackgroundInflammation plays a role in the pathogenesis of carotid atherosclerosis. Although previous data demonstrated an association between inflammatory biomarkers and stroke, there is no publication reporting the relation of neutrophil to lymphocyte (N/L) ratio with ischemic stroke. We aimed to analyze the predictive ability of N/L ratio in acute ischemic cerebrovascular disease. MethodsA total of 190 patients including 70 patients with first-ever atherothrombotic acute ischemic stroke (AAIS), 50 patients with transient ischemic attack, and 70 healthy subjects were enrolled in this study. We analyzed the values of white blood cell (WBC), N/L ratio, C-reactive protein (CRP), gamma-glutamyltransferase (GGT), homocysteine (HCY), mean platelet volume (MPV) in patients with ischemic group and compared those with control individuals. ResultsWBC, CRP, HCY, N/L ratio were found to increase significantly in AAIS patients than the controls (P < 0.001). With respect to mortality, there were no significant differences between the values of CRP, GGT, HCY, and MPV in patients with AAIS. However, WBC and N/L ratio values were found to increase significantly in dead patients (P = 0.024 and P = 0.029, respectively). A comparison of receiver operating characteristic curves among WBC, CRP, GGT, HCY, MPV, and N/L ratio variables was made. No significant differences were obtained between area under curve values (P > 0.05). A cut-off value of 4.1 for N/L ratio was detected in predicting mortality with a sensitivity of 66.7% and a specificity of 74.1% ( = 0.299, P = 0.006). ConclusionsThese findings support the role of N/L ratio as a simple inexpensive and readily available marker of prognosis in acute ischemic stroke. (C) 2013 Wiley Periodicals, Inc.

Summary: Activation of the HPA axis through an ITT shows distinct responses between males and females, with males showing activation of multiple transcripts in the hypothalamus and pituitary within 10 minutes, while females respond through the pituitary and adrenal immediately and sustainably for over 40 minutes. MicroRNA alterations precede mRNA responses in the HPA axis, with robust changes in specific transcripts suggesting differential transcriptional control between males and females via signaling in the pituitary and blood. The HPA axis responses to ITT involve complex regulatory signaling pathways of miRNAs and mRNAs, highlighting key markers in the stress-induced dynamics of the HPA, HPG and GH axes.

Summary: This study evaluated the demographic, clinical, and laboratory findings of symptomatic and asymptomatic patients with COVID-19. The findings showed that asymptomatic infections play a significant role in the spread of the disease. Therefore, it is crucial to identify and quarantine asymptomatic patients to prevent transmission and allow for early diagnosis of pre-symptomatic cases.

Patterns of COVID-19-related headache: A cross-sectional study

Summary: This study investigated the characteristics of COVID-19-related headache and their relationship with clinical severity. The results showed that bilateral, prolonged, moderate to severe headaches that were resistant to analgesics were more frequent in COVID-19 infected patients. Older age, high body mass index, and low education level were associated with greater severity of the disease. Female sex and being a healthcare worker were more common in milder cases.

Plasma biomarkers of brain injury in COVID-19 patients with neurological symptoms

Summary: This study examined the plasma levels of biomarkers GFAP and S100B in relation to persistent neurological symptoms in COVID-19 patients. The results showed that elevated GFAP levels were associated with disease severity but not with neurological symptoms, while high serum S100B levels were associated with multiple neurological symptoms.

Summary: This study found a significant relationship between neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) with neurological symptoms (NS) in COVID-19 patients. Advanced age, higher body mass index, and comorbidities were more common in severe cases. However, in the multivariate model, NLR and PLR were not independently associated with disease severity.

Role of vitamin D in the association between pre-stroke sleep quality and poststroke depression and anxiety

Summary: This study investigated the association between serum vitamin D levels, pre-stroke sleep quality, and the occurrence of poststroke depression and anxiety in acute ischemic stroke patients. The results showed that decreased serum vitamin D levels at admission were associated with a higher risk for poststroke depression and anxiety in patients with poor pre-stroke sleep quality during the early stages of acute ischemic stroke.
