Summary: Acute myocardial infarction is a leading cause of death worldwide. This study investigated the metabolic changes in serum during ischemia and reperfusion injury in a swine model, aiming to identify metabolomic biomarkers for myocardial injury at different phases. The results showed distinct profiles of serum metabolites during ischemia and early and late reperfusion, with impaired mitochondrial function and amino acid modifications during ischemia, and changes in branched-chain amino acid metabolites and bile acid pathway derivatives during reperfusion.

Summary: Cardiac remodeling is the primary pathological feature of chronic heart failure. Our study found that metabolic and structural remodeling of mitochondrion played a key role in ischemic heart disease in the early stages of ischemia. Purine metabolism may be a potential biomarker for early ischemia diagnosis.

Summary: This study aimed to explore the protective mechanism of Wogonoside (Wog) on acute myocardial ischemia (AMI) rats. The results showed that Wog effectively prevented ST-segment elevation in AMI rats, reduced myocardial infarct size and heart weight index, alleviated cardiac histological damage, and corrected disturbed metabolic profile. The integrated analysis of network pharmacology and metabolomics revealed that Wog regulated multiple metabolic biomarkers, targets, and pathways, suggesting its therapeutic potential for AMI.

Summary: Fatty acid metabolism aberrance occurred during AMI. S. miltiorrhiza and P. lobata play vital roles in the anti-inflammatory and anti-apoptotic action partially by regulating FFA levels. Our findings revealed potential novel clinical FFAs for predicting AMI and extended the insights into the compatible principle of XKS in which S. miltiorrhiza and P. lobata can potently modulate FFA metabolism.
