Frailty and Post-hospitalization Outcomes in Patients With Heart Failure With Preserved Ejection Fraction

Parag Goyal, Brian Yum, Pedram Navid, Ligong Chen, Dae H. Kim, Jason Roh, Byron C. Jaeger, Emily B. Levitan

Summary: The study aimed to identify and rank the importance of comorbid conditions associated with post-hospitalization outcomes of HFpEF patients. Age and frailty were found to be the most important predictors for mortality, while frailty was also crucial for predicting rehospitalization and the composite outcome of mortality or all-cause rehospitalization. Frailty was identified as a significant predictor of mortality and rehospitalization in older adults with HFpEF.

Sacubitril/Valsartan Initiation Among Veterans Who Are Renin-Angiotensin-Aldosterone System Inhibitor Naive With Heart Failure and Reduced Ejection Fraction

April F. Mohanty, Emily B. Levitan, Jordan B. King, John A. Dodson, Orly Vardeny, James Cook, Jennifer S. Herrick, Tao He, Olga Patterson, Patrick R. Alba, Patricia A. Russo, Engels N. Obi, Michelle E. Choi, James C. Fang, Adam P. Bress

Summary: The initiation rate of sacubitril/valsartan for HFrEF patients in the Veterans Administration increased gradually in the 4 years following the approval by the FDA. Compared with ACEI or ARB initiators, those initiating sacubitril/valsartan had fewer baseline cardiovascular comorbidities and the lowest proportion achieving >= 50% target daily dose at the 6-month follow-up. Identifying reasons for the lower follow-up dosing of sacubitril/valsartan could aid in supporting guideline recommendations and quality improvement strategies for HFrEF patients.

Summary: This study examined the ambulatory follow-up of patients with comorbid diabetes and heart failure after hospital discharge. The results showed that more than half of the patients did not receive guideline-concordant postdischarge care. Non-White and Hispanic/Other patients were less likely to receive recommended postdischarge care.

Summary: The study found that midlife cognitive activity was associated with late-life cognitive activity, but did not impact cognitive function or overall function. Adjusted analyses yielded similar results. However, using more comprehensive measures of cognitive activity may better detect such an association.
