Summary: Chronic pancreatitis is often associated with alcohol use, smoking, or genetic risk factors. Common symptoms include recurrent abdominal pain or pancreatitis, and characteristic imaging findings include pancreatic stones, dilated ducts, and atrophy. Complications include pseudocysts, biliary strictures, exocrine and endocrine pancreatic insufficiency, bone loss, and pancreatic cancer.

Summary: Exocrine pancreatic insufficiency (EPI) is caused by primary pancreatic disease or impaired exocrine pancreatic function, with chronic pancreatitis being the most common cause. Other causes include pancreatic tumors, pancreatic surgery, and cystic fibrosis. Pancreatic enzyme replacement therapy is recommended for clinically evident steatorrhea, weight loss, or symptoms of maldigestion and malabsorption associated with EPI.

Summary: Vitamin D metabolic disorder is implicated in both acute and chronic pancreatitis, with studies showing its anti-inflammatory and anti-fibrotic effects through binding with VDR. Further research is needed to establish the protective role of vitamin D in pancreatitis, but current evidence suggests that assessing and supplementing vitamin D levels are important strategies for management.

Summary: This study aimed to assess vitamin D status and determinants of vitamin D insufficiency in patients with COPD in Southwestern Sweden. The results showed that vitamin D insufficiency is common in patients with COPD and is related to male gender, smoking habits, lack of supplements containing vitamin D, and the winter season for blood sampling.

Summary: In patients with alcoholic chronic pancreatitis, advanced pancreatic burnout is closely associated with liver cirrhosis, disease duration, and rate of calcification, but not with the amount of alcohol consumption. The liver appears to be more vulnerable to alcohol damage compared to the pancreas.

Summary: Chronic alcohol consumption is associated with both chronic pancreatitis and liver cirrhosis. The prevalence of chronic pancreatitis and low fecal elastase in patients with decompensated liver disease was investigated in this study. The results showed that a significant proportion of patients had exocrine pancreatic insufficiency and chronic pancreatitis.

Summary: Chronic pancreatitis patients often have exocrine pancreatic insufficiency (EPI), which may lead to fat-soluble vitamin deficiencies. While vitamin D deficiency is common in the general population, deficiencies in vitamins A, E, and K are more prevalent in patients with chronic pancreatitis. It is uncertain whether routine monitoring of fat-soluble vitamin status should be done in all chronic pancreatitis patients or limited to those with EPI.

Summary: Patients with chronic pancreatitis (CP) are at increased risk of developing pancreatic exocrine insufficiency (PEI) and subsequently urinary oxalate stones. This study investigated the incidence and risk factors for nephrolithiasis in a Swedish cohort of CP patients. The results showed that 6.5% of definite CP patients were diagnosed with kidney stones, with PEI, increased BMI, and male sex being identified as independent risk factors.

Summary: This study developed and validated CT and radiomics nomograms to differentiate mass-forming chronic pancreatitis (MFCP) from pancreatic ductal adenocarcinoma (PDAC). The nomograms showed good differentiation ability in patients with chronic pancreatitis and have the potential to refine the management of pancreatic masses in these patients.

Summary: Pancreatic Ductal Adenocarcinoma (PDAC) is an aggressive disease that can alter the nutritional status of patients, leading to malnutrition and muscle wasting. The ideal way to measure these changes and identify them early to prevent complications or treatment discontinuation is still relatively unexplored. Nutritional assessment is crucial in PDAC patients, and bioimpedance vector analysis and radiological assessment of body composition appear to be more accurate in predicting clinical outcomes and postoperative complications.

Summary: The REAPPEAR study aims to investigate whether a combined patient education and cessation programme for smoking and alcohol prevents acute recurrent pancreatitis (ARP) due to alcohol and/or tobacco abuse. The study consists of a randomised controlled trial and a prospective cohort study. Participants will receive standard intervention and laboratory testing, and interventions based on laboratory results. The primary endpoint is the 2-year all-cause recurrence rate of ARP and/or all-cause mortality.

Summary: This systematic review and meta-analysis assessed the prevalence of endocrine and exocrine insufficiency in patients with autoimmune pancreatitis (AIP) and the impact of glucocorticoid (GC) therapy on pancreatic function. The study found that a large proportion of AIP patients displayed both endocrine and exocrine insufficiency at diagnosis. Long-term use of GCs was associated with an increased incidence of diabetes.

Summary: Upfront resection is less common for pancreatic ductal adenocarcinoma, and neoadjuvant treatments are increasingly used. However, complications such as jaundice, gastric outlet obstruction, and acute cholecystitis can lead to chemotherapy discontinuation, making effective management of these complications crucial. t-EUS has a high efficacy-invasiveness ratio compared to other methods, making it a powerful tool against chemotherapy discontinuation and potentially increasing subsequent resectability. Restricting t-EUS in potential surgical candidates is unsupported by evidence.

Summary: Needle-tract seeding (NTS) is a rare but serious complication of Endoscopic Ultrasound-guided biopsy of pancreatic adenocarcinoma. Our study aimed to deepen the understanding of NTS and highlight therapeutic approaches. The findings showed that NTS has a low incidence and usually occurs late, but aggressive treatment can lead to good results.

Summary: Thermal ablation under endoscopic ultrasound (EUS)-guidance has shown potential to enhance immune response in pancreatic ductal adenocarcinoma (PDAC). In a recent trial, EUS-guided ablation with HybridTherm Probe (HTP) in combination with chemotherapy improved disease progression compared to chemotherapy alone. This study explored the effects of EUS-HTP on systemic immune response in PDAC and found that it selectively affected immunological predictors of poor outcome.
