Diagnosis and Treatment of Subclinical Hypercortisolism

Context: Subclinical hypercortisolism (SH) is a condition of biochemical cortisol excess without the classical signs or symptoms of overt hypercortisolism. It is thought to be present in the 5-30% of patients with incidentally discovered adrenal mass (adrenal incidentalomas), which in turn are found in 4-7% of the adult population. Therefore, SH has been suggested to be present in 0.2-2.0% of the adult population. Some studies suggested that this condition is present in 1-10% of patients with diabetes or established osteoporosis. The present manuscript reviews the literature on diagnostic procedures and the metabolic effect of the recovery from SH. Evidence Acquisition: A PubMed search was used to identify the available studies. The most relevant studies from 1992 to November 2010 have been included in the review. Evidence Synthesis: The available data suggest that SH may be associated with chronic complications, such as hypertension, diabetes mellitus, overweight/obesity, and osteoporosis. The available intervention studies suggest that the recovery from SH may lead to the improvement of hypertension and diabetes mellitus. A retrospective study suggests that this beneficial effect could be predicted before surgery. Conclusions: SH is suggested to be associated with some chronic complications of overt cortisol excess. Recovery from this condition seems to improve these complications. However, a large, prospective, randomized study is needed to confirm this hypothesis and to establish the best diagnostic approach to identify patients with adrenal incidentalomas who can benefit from surgery. Pathophysiology of Mild Hypercortisolism: From the Bench to the Bedside

Summary: Mild hypercortisolism refers to abnormal cortisol secretion without the overt manifestations of Cushing's syndrome. It is commonly found in patients with adrenal incidentalomas and incidentally discovered pituitary tumors. The pathogenesis of hypercortisolism of pituitary origin is still unclear, while advances have been made in understanding the genetic background of adrenal adenomas causing mild hypercortisolism. The clinical consequences of mild hypercortisolism include bone fragility, arterial hypertension, subclinical atherosclerosis, cardiovascular remodeling, dyslipidemia, glucose metabolism impairment, visceral adiposity, infections, muscle damage, mood disorders, and coagulation.

Effects of the therapy shift from cortisone acetate to modified-release hydrocortisone in a group of patients with adrenal insufficiency

Summary: This study aimed to assess clinical and biochemical modifications in patients shifted from cortisone acetate (CA) to modified-release hydrocortisone (MRH). The results showed that patients shifted to MRH treatment had improved blood pressure control and symptoms of over- or undertreatment, but no significant changes in glucose metabolism or total cortisol exposure were observed. Therefore, a dose reduction may be needed when shifting from CA to MRH.

ACTH Stimulation Test for the Diagnosis of Secondary Adrenal Insufficiency: Light and Shadow

Summary: Secondary Adrenal Insufficiency (SAI) is a condition characterized by low ACTH secretion due to hypothalamus or pituitary disease or injury. The evaluation of SAI is challenging due to non-specific symptoms, rarity, and pitfalls associated with laboratory tests. The gold standard test for assessing the HPA axis is the insulin tolerance test, but the ACTH stimulation test is safer and well-tolerated, although its diagnostic accuracy varies.

Bone Disease in Long-Term Lung Transplant Survivors

Summary: Based on our study, the skeletal outcomes in long-term survivors (>5 years) after lung transplantation were similar between patients with cystic fibrosis (CF) and those with non-cystic fibrosis (nCF), with a lower incidence of fractures compared to the first two years post-transplant. Bone mineral density and trabecular bone score remained stable after the second year post-transplant.

Summary: This study evaluated the hematological alterations in benign adrenocortical tumors with different degrees of cortisol excess. It found that the degree of cortisol excess was correlated with an increase in leukocytes, neutrophils, and monocytes. Additionally, the neutrophil-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, and systemic immune-inflammation index were altered in patients with mild autonomous cortisol secretion (MACS) and adrenal Cushing's syndrome.
