Cost-Effectiveness and Cost-Utility Analysis of Spinal Cord Stimulation in Patients With Failed Back Surgery Syndrome: Results From the PRECISE Study

ObjectiveTo assess the cost-effectiveness and cost-utility of Spinal Cord Stimulation (SCS) in patients with failed back surgery syndrome (FBSS) refractory to conventional medical management (CMM). Materials and MethodsWe conducted an observational, multicenter, longitudinal ambispective study, where patients with predominant leg pain refractory to CMM expecting to receive SCS+CMM were recruited in 9 Italian centers and followed up to 24 months after SCS. We collected data on clinical status (pain intensity, disability), Health-Related Quality-of-Life (HRQoL) and on direct and indirect costs before (pre-SCS) and after (post-SCS) the SCS intervention. Costs were quantified in Euro 2009, adopting the National Health Service's (NHS), patient and societal perspectives. Benefits and costs pre-SCS versus post-SCS were compared to estimate the incremental cost-effectiveness and cost utility ratios. Results80 patients (40% male, mean age 58 years) were recruited. Between baseline and 24 months post-SCS, clinical outcomes and HRQoL significantly improved. The EQ-5D utility index increased from 0.421 to 0.630 (p < 0.0001). Statistically significant improvement was first observed six months post-SCS. Societal costs increased from Euro6600 (pre-SCS) to Euro13,200 (post-SCS) per patient per year. Accordingly, the cost-utility acceptability curve suggested that if decision makers' willingness to pay per Quality-Adjusted-Life-Years (QALYs) was Euro60,000, SCS implantation would be cost-effective in 80% and 85% of cases, according to the NHS's and societal point of views, respectively. ConclusionsOur results suggest that in clinical practice, SCS+CMM treatment of FBSS patients refractory to CMM provides good value for money. Further research is encouraged in the form of larger, long-term studies.

Real-world costs and dynamics of surveillance in patients who underwent surgery for low-risk branch duct intraductal papillary mucinous neoplasms

Summary: This study evaluated the rate of correct indication for pancreatectomy in low-risk BD-IPMN patients who underwent surveillance. Only 23.8% had a high grade dysplasia, while 59.7% had a low grade dysplasia and 14.9% had invasive cancer among those who were operated. The mean surveillance cost was 194.9 euros per patient-year, with higher costs in the correct surgery group compared to the overtreatment and undertreatment groups. The rate of appropriate surgery in BD-IPMNs under surveillance is low.

Summary: This study assessed the cost-effectiveness of a treat-to-target strategy based on clinical signs and faecal calprotectin levels in patients with mild-moderate ulcerative colitis. The results showed that this approach increased the time spent in clinical remission and reduced the number of relapses compared to a strategy based solely on symptoms. However, it was also associated with higher costs.

The study also revealed that individuals who experienced the same post-vaccination symptoms after the second dose were more likely to report AEFI after the third dose. These findings provide important data for informing booster vaccination strategies in healthcare workers and the adult population.

Summary: This study aimed to assess the cost and cost-effectiveness of tucatinib when used in combination with trastuzumab and capecitabine compared to TDM-1 for HER2(+) breast cancer. The findings suggest that TCX is cost-effective at a willingness to pay of 60,000 EUR, while TDM-1 is more cost-effective at a willingness to pay of 20,000 EUR. The results can be used to ensure the affordability of tucatinib in Italy and provide a pricing threshold for other countries.

Summary: The European Liver and Intestine Transplant Association (ELITA) held a Consensus Conference involving experts worldwide to update guidelines on HBV prophylaxis in liver transplant candidates and recipients. A simulation model was developed to compare the economic impact of implementing the new ELITA guidelines. The model showed that the guidelines could save around euro 235.65 million after 5 years and euro 540.73 million after 10 years. These savings could be used to allocate resources to different needs.
