A 21-year-old woman presented with coma after 5 days of fever, gait ataxia, and somnolence. Brain MRI showed lesions in medial thalami, caudate heads, and periaqueductal region (figure). Hyperlactatemia was present; serum thiamine levels were normal.

Summary: The study compared the onset of progressive motor impairment in patients with highly restricted versus unlimited CNS lesion burden. It found that the age of motor-progression onset is similar, but paradoxically earlier, in cohorts with highly restricted CNS lesion burden. The critical demyelinating lesion is a major contributor to motor-progression onset.

Summary: Despite aggressive therapeutic regimens, glioblastoma (GBM) remains a deadly brain tumor with significant resistance to radiotherapy and chemotherapy. Proton boron capture therapy (PBCT) shows promise in increasing therapeutic effectiveness and improving the quality of life of GBM patients, by utilizing radiosensitizing agents to enhance the effects of proton beam. A preclinical evaluation of PBCT in a GBM model demonstrated increased cell death and mitophagy, validating PBCT as a scalable strategy to treat GBM.

Summary: People with critical demyelinating lesion-induced progressive motor impairment typically have elevated levels of intrathecal IgG synthesis. Their clinical and radiological presentation is similar regardless of abnormal cerebrospinal fluid findings, indicating that this is a valid manifestation of progressive demyelinating disease.
