Transgenic mice overexpressing human G972R IRS-1 show impaired insulin action and insulin secretion.

Molecular scanning of human insulin receptor substrate (Irs) genes revealed a single lrs1 prevalent variant, a glycine to arginine change at codon 972 (G972R); previous in vitro studies had demonstrated that the presence of this variant results in an impaired activation of the insulin signalling pathway, while human studies gave controversial results regarding its role in the pathogenesis of insulin resistance and related diseases. To address in vivo impact of this IRS-1 variant on whole body glucose homeostasis and insulin signalling, we have generated transgenic mice overexpressing it (Tg972) and evaluated insulin action in the liver, skeletal muscle and adipose tissue and assessed glucose homeostasis both under a normal diet and a high-fat diet. We found that Tg972 mice developed age-related glucose and insulin intolerance and hyperglycaemia, with insulin levels comparatively low. Glucose utilization and insulin signalling were impaired in all key insulin target tissues in Tg972 mice. There were no differences in pancreatic morphology between Tg972 and wild-type mice, however when insulin secretion was evaluated in isolated islets, it was significantly reduced in Tg972 mice islets at any glucose concentration tested. Under a high-fat diet, Tg972 mice had increased body and adipose tissue weight, and were more prone to develop diet-induced glucose and insulin intolerance. So, we believe that Tg972 mice may represent a useful model to elucidate the interaction between genetic and environmental factors in insulin resistance pathogenesis. Furthermore, they may become an important tool to test novel tailored therapies.

Insulin receptor activation by proinsulin preserves synapses and vision in retinitis pigmentosa

Summary: Synaptic loss, neuronal death, and circuit remodeling are common features of central nervous system neurodegenerative disorders. Impaired insulin receptor signaling has been linked to retinitis pigmentosa (RP). Decreased levels of the insulin receptor and its downstream effector phospho-S6 were observed in a mouse model of RP, along with abnormal synapses between photoreceptors and other cells. Gene therapy using proinsulin restored retinal insulin receptor signaling, preserved synaptic connectivity, and prolonged visual function in RP mice.

Summary: This study found that individuals with overweight and obesity have higher protein levels of SGLT-1, GLUT-2, and GLUT-5 in the duodenum compared to lean individuals. These protein levels are positively correlated with body weight, waist circumference, postprandial glucose, insulin levels, and insulin resistance. The results suggest that increased duodenal SGLT-1 and GLUT-5 abundance in overweight and obese individuals may contribute to higher postprandial glucose concentrations, insulin resistance, and hyperinsulinemia.

Summary: This study investigated the effects of a natural mixture of polyphenols and micronutrients (A5(+)) on counteracting white adipogenesis and promoting fat browning. The results showed that A5(+) significantly reduced lipids accumulation, inhibited cellular proliferation, and decreased the release of pro-inflammatory cytokines. Additionally, A5(+) promoted fat browning and fatty acid oxidation by increasing the expression of genes related to brown adipose tissue.

Summary: This retrospective study examined cardiovascular risk factors and specific coronary findings in high-risk immigrant groups from Eastern Europe, the Middle East and North Africa, and South Asia. The results showed variations in cardiovascular disease among different ethnicities, with South Asians being particularly high-risk and other groups having lower cardiovascular risk factors. This study supports the role of genetic influence in these different ethnic communities.

Summary: This review summarizes the impact of influenza on vulnerable populations, emphasizing the benefits of influenza vaccination in preventing severe illness and complications. The efficacy, effectiveness, and safety of vaccines suitable for older adults in Italy are also reviewed. Although high-dose and adjuvanted influenza vaccines have been developed to enhance immune responses in older adults, vaccination coverage in high-risk populations remains suboptimal. Healthcare professionals play an important role in increasing vaccination rates.

Summary: Breakthrough infections (BIs) in rheumatoid arthritis (RA) patients after a COVID-19 vaccination booster dose were analyzed. Older age, receiving conventional synthetic disease modifying antirheumatic drugs (csDMARDs), and higher neutralising antibody titres were identified as protective factors, while anti-IL6R treatment and anti-CD20 therapy increased the risk. The incidence of hospitalization for BIs was significantly higher in RA patients compared to healthcare workers.

GLP-1RAs and cardiovascular disease: is the endothelium a relevant platform?

Summary: High blood sugar strongly affects endothelial function and activation, increasing the risk of atherosclerotic cardiovascular disease. Glucagon-like peptide 1 receptor agonists (GLP-1RA), a class of drugs aimed at lowering blood glucose levels, show potential in improving endothelium damage and cardiovascular disease progression. They have direct favorable actions on coronary vascular endothelium, such as reducing oxidative stress and increasing nitric oxide. Further research is needed to understand the specific role of GLP-1RA in cardiovascular disease management and identify cellular targets involved in protective signal transmission.
