Summary: Objective symptoms of gastroparesis and functional dyspepsia overlap, and using egg protein substitute to measure gastric emptying can help reclassify patients. This study assessed the inter-individual and intra-individual coefficients of variation in gastric emptying in symptomatic patients with gastroparesis or functional dyspepsia. The results suggest that the 320 kcal (30% fat) gastric emptying scintigraphic test provides consistent diagnoses in the majority of patients.

Summary: Patients with delayed and normal gastric emptying had similar symptom profiles during gastric emptying scintigraphy. Idiopathic patients with symptoms of gastroparesis had less gastric retention but more symptoms after meals and water load satiety test compared with diabetic patients. Decreasing water consumption during water load satiety test was associated with increased symptoms, suggesting impaired accommodation and visceral hypersensitivity play a role in postprandial symptoms in gastroparesis.

Summary: Gastroparesis, a disorder characterized by delayed stomach emptying, has limited treatment options. Gastric electrical stimulation (GES) has shown promise in treating the signs and symptoms of gastroparesis by stimulating the stomach with high-frequency electrical impulses. In this case, a 43-year-old woman with refractory gastroparesis underwent laparoscopic implantation of a GES device. Further studies are needed to improve patient selection, technique, and long-term outcomes of GES. Patients with refractory gastroparesis who have not responded to traditional therapy should be considered for GES, with treatment decisions based on individual clinical presentation and patient preferences.

Summary: This study quantitatively examined the gastric mucosal innervation density (MID) and its relationship with gastroparesis symptoms and gastric emptying in patients with diabetes. The study found that the gastric MID in patients with diabetes was significantly lower compared to healthy controls, and this reduction was correlated with gastroparesis symptoms and glycemic control. The study also showed prolonged gastric emptying in patients with diabetes. These findings provide important pathology and functional biomarkers for diabetic gastroparesis and shed light on the underlying pathophysiology.
