Diagnosis and classification of Goodpasture's disease (anti-GBM)

Goodpasture's disease or anti-glomerular basement membrane disease (anti-GBM-disease) is included among immune complex small vessel vasculitides. The definition of anti-GBM disease is a vasculitis affecting glomerular capillaries, pulmonary capillaries, or both, with GBM deposition of anti-GBM auto-antibodies. The disease is a prototype of autoimmune disease, where the patients develop auto-antibodies that bind to the basement membranes and activate the classical pathway of the complement system, which start a neutrophil dependent inflammation. The diagnosis of anti-GBM disease relies on the detection of anti-GBM antibodies in conjunction with glomerulonephritis and/or alveolitis. Overt clinical symptoms are most prominent in the glomeruli where the inflammation usually results in a severe rapidly progressive glomerulonephritis. Despite modern treatment less than one third of the patients survive with a preserved kidney function after 6 months follow-up. Frequencies vary from 0.5 to 1 cases per million inhabitants per year and there is a strong genetic linkage to HLA-DRB1*1501 and DRB1*1502. Essentially, anti-GBM disease is now a preferred term for what was earlier called Goodpasture's syndrome or Goodpasture's disease; anti-GBM disease is now classified as small vessel vasculitis caused by in situ immune complex formation; the diagnosis relies on the detection of anti-GBM in tissues or circulation in conjunction with alveolar or glomerular disease; therapy is effective only when detected at an early stage, making a high degree of awareness necessary to find these rare cases; 20-35% have anti-GBM and MPO-ANCA simultaneously, which necessitates testing for anti-GBM whenever acute test for ANCA is ordered in patients with renal disease. Bone Marrow Neutrophils of Multiple Myeloma Patients Exhibit Myeloid-Derived Suppressor Cell Activity

Summary: The study showed that NDGs from bone marrow of multiple myeloma patients suppress T-cell responses like MDSCs, while blood NDGs from both patients and healthy individuals can inhibit T-cell proliferation and IFN-gamma production. The inhibitory effect of NDGs is dependent on reactive oxygen species production and contact with activated T-cells, indicating the importance of their role in regulating T-cells. Bone marrow NDGs from MM patients exhibit MDSC-like function, which could play a significant role in promoting malignant plasma cell growth in MM patients.

These antibodies bind to the central region of the M protein and promote immune functions. The concept of dual-Fab cis binding allows access to normally non-opsonic regions for antibody targeting.

Using imlifidase to elucidate the characteristics and importance of anti-GBM antibodies produced after start of treatment

Summary: This study found that the characteristics of anti-GBM antibodies have an important impact on treatment outcomes in glomerulonephritis patients. The rebound of anti-GBM antibodies during treatment is associated with a worse prognosis, especially antibodies directed against the E-B epitope. Therefore, all means should be used to eliminate anti-GBM antibodies.

Evaluation of the ACR/EULAR 2022 criteria for classification of ANCA-associated vasculitis in a population-based cohort from Sweden

Summary: The ACR/EULAR 2022 criteria for ANCA-associated vasculitides (AAV) classification were evaluated and compared with the European Medicines Agency (EMA) algorithm and classification based only on ANCA serology. The results showed that the EMA and ACR/EULAR classification gave similar results, but a small proportion of patients could not be classified or fell into two categories. The ANCA-based classification showed high agreement with ACR/EULAR criteria.

A population-based study on the incidence and aetiology of infectious complications in peritoneal dialysis in South Sweden

Summary: This study investigated the incidence, trend, etiology, and antimicrobial susceptibility of PD-associated peritonitis and catheter-related infections in South Sweden between 2011-2020 among PD patients. The study found a stable incidence rate of PD-peritonitis, with gram-positive bacteria being the most common cause and limited antibiotic resistance. Monitoring the etiology, incidence, and resistance rates of PD-associated infections is important for empirical antibiotic regimens and prevention.

Isolation and characterisation of PR3-specific B cells and their immunoglobulin sequences
